Praha. 2020. 61 s. Tématem bakalářské práce je intrahepatální cholestáza v těhotenství z pohledu PA. Intrahepatální cholestáza v těhotenství je nejčastějším reverzibilním onemocněním jater vázaným pouze na těhotenství se spontánním vymizením v šestinedělí. Vzhledem k vysokým rizikům pro plod zapříčiněných cholestatickou hepatózou, je nutné, abychom byli o této problematice i o lékařské a ošetřovatelské péči poskytované porodníky a PA obeznámeni. ICP se relativně snadno identifikuje a máme pro ni i jasný management péče. Těhotenská cholestáza je pro plod velice nebezpečná a nese s sebou velká rizika, větší, než jaká způsobují těhotné ženě. Teoretická část práce popisuje těhotenskou cholestázu z medicinského hlediska na základě definice, diagnostiky, etiologie, poté vlivu onemocnění na gravidní ženu i na plod a také zahrnuje klasifikaci ICP a management péče v těhotenství, při porodu a po něm. Následně popisuje specifickou ošetřovatelskou péči, kterou poskytuje PA ženě s cholestázou v těhotenství. Praktická část práce je zpracována jako kazuistika těhotné ženy s těžkou formou ICP. Cílem teoretické části je nalézt a nastudovat dostupné materiály z odborné literatury na dané téma. Cíl praktické části je popsat a zhodnotit péči PA o ženu s intrahepatální cholestázou v těhotenství.

This bachelor thesis focuses on intrahepatic cholestasis of pregrancy from the midwife’s perspective. Intrahepatic cholestasis of pregnancy is the most common, reversible liver disease closely connected with pregnancy and spontaneously resolving in puerperium. Given the high risks caused by ICP to the fetus, it is essential to be aware of this issue and of the medical and nursing care provided by obstetricians and midwives. ICP is relatively easy to identify and we have an accurate management of care for it. Pregnancy cholestasis is very dangerous for the fetus and constitutes a greater risk than it does to a pregnant woman. The theoretical part describes pregnancy cholestasis from the medical point of view on the basis of definition, diagnostics, etiology, then the influence of the disease on the pregnant woman and the fetus and also includes ICP classification and management of care during pregnancy, delivery and afterwards. Then, it describes the specific nursing care provided by a midwife to a woman with cholestasis during pregnancy. The practical part of the thesis is a case study of a pregnant woman with a more severe form of ICP. The aim of the theoretical part is to find and study available materials from specialized literature on the topic. The aim of the practical part is to describe and evaluate the care of a midwife for a woman suspected of intrahepatic cholestasis during pregnancy.
