Nursing Care of a Patient after Liver Transplantation

Abstract:

Tématem bakalářské práce je ošetřovatelská péče o pacienta po transplantaci jater. Teoretická část krátce popisuje anatomii a fyziologii jater a žlučových cest, jaterní onemocnění, komplikace, jejich diagnostiku. Dále v teoretické části sleduje popis HCC jeho klinický obraz, diagnostiku, léčbu. Zvláštní kapitola je věnovaná transplantaci jater. Praktická část popisuje celkovou ošetřovatelskou péči o konkrétního pacienta po transplantaci jater, která probíhala na Klinice hepatogastroenterologie na jednotce intenzivní péče Institutu klinické a experimentální medicíny. IKEM je centrem, kde se provádí transplantace. U pacientů s HCC je transplantace jater je obrovskou nadějí na vyléčení. Ošetřovatelské posouzení bylo vypracováno za využití koncepčního modelu funkčního zdraví dle Virginie Hendersonové a stanovení ošetřovatelských problémů a ošetřovatelských diagnóz dle NANDA taxonomie 2018-2020. Během hospitalizace byla ošetřovatelská péče kompletní, s uspokojením všech potřeb pacienta, potřeby jistoty, bezpečí a nezávislostí, soběstačností (bio-psycho-socio-spirituálních). Jako součást práce byla zpracována doporučení pro praxi, která by měla pomáhat všeobecným sestrám v jejich práci, rodině pacienta v pomoci blízké osobě, pacientovi v rychlém uzdravení.

Abstract:

The topic of the bachelor's thesis is nursing care of a patient after a liver transplantation. The theoretical part briefly describes the anatomy and physiology of the liver and bile ducts, liver diseases, complications, and their diagnosis. Further, in the theoretical part, the description of HCC follows its clinical picture, diagnosis, and treatment. A special chapter is devoted to liver transplantation. The practical part describes the overall nursing care of a specific patient after a liver transplantation, which took place at the Hepatogastroenterology Clinic in the intensive care unit of the Institute of Clinical and Experimental Medicine. IKEM is the center where the transplantation is carried out. For patients with HCC, liver transplantation is a huge hope for a cure. The nursing assessment was developed using the conceptual model of functional health according to Virginia Henderson and the determination of nursing problems and nursing diagnoses according to the NANDA taxonomy 2018-2020. During the hospitalization, the nursing care was complete, with the satisfaction of all the patient's needs, the need for security, safety and independence, self-sufficiency (bio-psycho-socio-spiritual). As part of the works, recommendations for practice were developed, which should help general nurses in their work, the patient's family in helping a loved one, the patient in a quick recovery.
