Rehabilitation of swallowing disorders in patients with stroke: effectiveness of the electrical stimulation with Vital-stim.

Abstract:

ABSTRACT Title: REHABILITATION OF SWALLOWING DISORDERS IN STROKE PATIENTS: VITAL ELECTROSTIMULATION - STIM EFFICACY. Research type: Single Case Study. Objective: verify the feasibility and effectiveness of dysphagia rehabilitation using Vital Stim stimulator associated with traditional rehabilitative treatment in stroke Patients. Materials and methods: Sample studied: 1 male Patient, Age: 71 years. Inclusion criteria: neurogenic dysphagia after stroke severity with value . 4 according to the DOSS scale. Diagnosis: neurogenic dysphagia in bulbo-pontine ischemic stroke. Distance from event: less than 6 months from the acute event. In the case report is equal to 6 months. Method of feeding and hydration of the patient at the beginning of the study: totally by artificial enteral. Protocol assessment of dysphagia: - Clinical evaluation of swallowing team: (Bedside Swallowing Test Evaluation) - Radiological Assessment Instrument video fluoroscopy (VFSS) and fibro-endoscopy (FEES) Functional scale assessment of swallowing: - Pooling score - Penetration-Aspiration Scale - DOSS Dysphagia Outcome and Severity Scale - MASA Mann Assessment of Swallowing Ability - Self-evaluation Swallow quality of life (QOL-SWALs) Timing of assessments: - Pre-treatment - Post-treatment: after 8 weeks of treatment - Follow up: after 4 weeks after the end of treatment Dysphagia rehabilitation treatment: - Traditional rehabilitation exercises and electrostimulation with Vital Stim Portable: stimulation with 2 independent channels with application of 4-electrode - Timing of use: 1 session per day from 60 minutes to 5 days per week for 8 weeks. 10 Results: Recovery of feeding by mouth with a full adaptation of the semi-solid foods to a smooth consistency, in the absence of passage of food into the airway and pulmonary aspiration of intercurrent complications. Maintaining a good set of nutrition. Conclusions: The treatment of electrostimulation with Vital Stim seemed effective in enhancing the action of the classic rehabilitative treatment of dysphagia. Given the small sample size considered in this study (1 patient), it is not considered possible to draw general conclusions. The results observed and the concomitant lack of scientific papers and guidelines, open the possibility of continuing with the investigation of this area of rehabilitation, in particular with larger samples of the population.
