Influence of administration of a contrast medium on some selected functional parameters of the kidneys

Abstract:

Cílem práce bylo posoudit vliv podání kontrastní látky na některé vybrané funkční parametry ledvin a posoudit možnosti hodnocení markerů tubulárního poškození v praxi. Dalším cílem bylo porovnat dvě skupiny pacientů s rozvojem a bez rozvoje kontrastem indukované nefropatie s ohledem na přítomnost či absenci rizikových faktorů. Ve vlastní práci jsme sledovali 59 pacientů, u kterých bylo za hospitalizace provedeno vyšetření s intravenózním podáním kontrastní látky iomeprolu. Ve stanovených časových intervalech před a po podání kontrastní látky byly sledovány v séru a v moči vybrané biochemické parametry včetně markerů tubulární toxicity kontrastní látky N-acetyl-β-D-glukozaminidáza (NAG) a gama-glutamyltransferáza (GGT). Výsledky byly statisticky zpracovány. Prokázali jsme, že po podání nízkoosmolární kontrastní látky iomeprolu došlo k signifikantnímu nárustu NAG a GGT v moči. Nárust GGT byl časnější a dosahoval maximální hodnoty první den po podání kontrastní látky s následným poklesem. Hodnota NAG v moči dosáhla maxima dva dny po podání kontrastní látky. Neprokázala se korelace mezi objemem kontrastní látky, resp. obsahem jódu, a změnou tubulárních enzymů. Porovnáním skupiny s rozvojem a bez rozvoje kontrastem indukované nefropatie byl ve skupině s nefropatií zjištěn signifikantní rozdíl v parametrech, které jsou popisovány jako hlavní rizikové faktory rozvoje kontrastem indukované nefropatie (věk, preexistující renální poškození včetně diabetické nefropatie, množství kontrastní látky). Rovněž jsme jako rizikové faktory vzniku kontrastem indukované nefropatie identifikovali markery aterosklerózy, jako jsou celkový cholesterol, triacylglyceroly a kyselina močová.

Abstract:

The aim of the thesis was to assess the influence of administration of a contrast medium on some selected functional parameters of the kidneys and to assess the possibilities of evaluating markers of tubular damage in practice. A further aim of the thesis was to compare two groups of patients with and without the development of contrast-induced nephropathy with respect to the presence or absence of risk factors. In the thesis proper, we were was monitoring 59 patients who, during hospitalisation, were subjected to examination with intravenous administration of the contrast medium iomeprol. At fixed time intervals prior to and following the administration of the contrast medium, selected biochemical parameters were monitored in the serum and urine, including markers of tubular toxicity of the contrast medium N-acetyl-β-D-glucosaminidase (NAG) and gama-glutamyltransferase (GGT). The results were statistically processed. We have demonstrated that administration of the low-osmolar contrast medium iomeprol resulted in a significant rise of NAG and GGT in the urine. The rise in GGT was earlier and reached its maximum value on the first day after administration of the contrast medium with subsequent decline. The value of NAG in the urine reached its maximum two days after administration of the contrast medium. No correlation was demonstrated between the volume of the contrast medium and/or the content of iodine and the change of tubular enzymes. When comparing the group with and without the development of contrast-induced nephropathy, a significant difference was detected in the group with nephropathy in parameters which have been described as the main risk factors of the development of contrast nephropathy (age, pre-existing renal damage including diabetic nephropathy, amount of contrast medium). We have also identified markers of atherosclerosis, such as total cholesterol, triacylglycerols and uric acid, as risk factors for the development of contrast-induced nephropathy.
