Virulence factors in the genus Klebsiella and urinary tract infections

Abstract:

Urinary tract infections are the most common nosocomial infections, while 80 % of these infections is associated with indwelling urinary catheter. Catheter provides suitable place for bacterial adhesion and subsequent biofilm formation. Among genera frequently isolated from these infections belongs genus Klebsiella. Bacterial virulence factors play an important role in colonisation of urinary tract and urinary catheters, and also in bacterial resistance to the immune system of the host. In this study selected virulence factors were detected: biofilm, urease, siderophores production, β-lactamase and carbapenemase. I tested a collection of 315 strains belonging to genus Klebsiella, which were isolated from urinary catheters. Selected virulence factors were detected in K. pneumoniae, K. oxytoca and R. terrigena isolates. Statistically significant differences between species have been demonstrated only in the production of urease. However, significant differences were also seen within the species level, among individual isolates. The most important virulence factor was a biofilm, which was detected in 95,9 % of isolates. Urease enzyme was produced in a large number of isolates, but in relatively low levels and the production of siderophores was observed in only 25,4 % isolates. A large proportion of Extended-spectrum β-lactamase producers was found (29,6 %) and the AmpC β lactamase was produced by only 2 isolates. Carbapenemase production was not detected. Overall results shows that the most virulent was K. pneumoniae and the least virulent species was K. oxytoca.

Abstract:

Infekce močového traktu jsou nejčastějšími nozokomiálními infekcemi, přičemž až 80 % těchto infekcí vzniká v souvislosti se zavedením permanentního močového katétru. Umělý povrch katétru představuje vhodné místo pro bakteriální adhezi a následnou tvorbu biofilmu. Jedním z významných rodů izolovaných z těchto infekcí je rod Klebsiella. Důležitou roli hrají faktory virulence bakterií, pomocí kterých kolonizují tkáně močového traktu či močové katétry a odolávají imunitnímu systému hostitele. V práci byly detekovány vybrané faktory virulence včetně biofilmu, ureázy, sideroforů, β-laktamáz a karbapenemáz. Vyšetřovaných bylo celkem 315 izolátů Klebsiella spp. z močových katétrů. Vybrané virulenční faktory byly detekovány u K. pneumoniae, K. oxytoca a R. terrigena. Statisticky významné rozdíly mezi jednotlivými druhy byly prokázány jen v produkci ureázy, ale výrazné rozdíly byly zaznamenány i na úrovni kmenů. Nejvýznamnějším faktorem virulence byl biofilm, který byl detekován u 95,9 % izolátů. Enzym ureáza byl produkován u velkého procenta izolátů, ale v relativně malém množství a produkce sideroforů byla pozorována pouze u 25,4 % izolátů. Velký podíl byl producentů širokospektrých β laktamáz (29, 6%), β-laktamázy typu AmpC produkovaly jen 2 izoláty. Nebyla detekována produkce karbapenemáz. Z celkových výsledků vyplývá, že nejvíce virulentním druhem byla K. pneumoniae a nejméně virulentní byl druh K. oxytoca.
