Evaluation of short-term heart rate and blood pressure changes in patients with esential arterial hypertension

Abstract:

Cílem naší práce bylo zhodnotit rozdíly v krátkodobých změnách srdeční frekvence a krevního tlaku mezi zdravými subjekty a skupinou pacientů s nově diagnostikovanou lehkou esenciální arteriální hypertenzí. Vyšetřované soubory tvořilo 39 zdravých a 37 hypertoniků ve věkovém rozmezí 18-45 let. Protokol vyšetřování byl zvolen tak, aby zahrnoval jak klidové měření se spontánní dechovou frekvencí, tak i 4 různé typy excitací k hodnocení jejich vlivu na krátkodobé změny v cirkulaci: 1) řízené dýchání s dechovou frekvencí 6 a 20 dechů za minutu, 2) test na nakloněné rovině s řízenou dechovou frekvencí 6 a 20 dechů za minutu, 3) psychická zátěž, 4) fyzická zátěž. V průběhu měření se kontinuálně a časově synchronizovaně zaznamenávaly signály EKG, systémového krevního tlaku a impedanční kardiografie. Následně byly pomocí off-line analýzy vyhodnoceny 5ti minutové úseky měření, reprezentující ustálené podmínky (vzhledem k frekvenci dýchání, poloze těla, psychické a fyzické zátěži). Výsledné parametry ( celkem 14) byly použity pro stanovení základních odlišností funkce autonomního nervového systému a rozdílů v krátkodobé regulaci krevního oběhu mezi oběma testovanými soubory.

Abstract:

The objective of the study was to assess the differences between short-term variation in heart rate and blood pressure in healthy individuals and in a group of patients with newly diagnosed mild essential arterial hypertension. The study samples consisted of 39 healthy volunteers (men/women 28/11, aged 28.5± 8.7) and 37 patients with newly diagnosed mild essential arterial hypertension (men/women 23/14, aged 31.7 ± 9.2), between 18 and 45 years of age. The examination protocol was designed in a way to include both the resting measurement with spontaneous breathing frequency, and 4 different types of excitation to evaluate their impact on short-term variation in circulation: 1) controlled breathing at the rate of 6 to 20 breaths per minute, 2) head-up tilt table test with controlled breathing at the rate of 6 to 20 breaths per minute, 3) mental strain, 4) physical strain. During the measurement, ECG, systemic blood pressure and impedance cardiography signals were recorded in a continuous and time synchronised manner. Subsequently, 5 minute measurement intervals representing stabilised conditions (with respect to the frequency of breathing, body position, mental and physical strain) were evaluated using off-line analysis. The resulting parameters (14 in total) were used to define the basic differences between the functioning of the autonomic nervous system and short-term regulation of blood circulation of the two tested groups.
