Abstract:

Cytogenetická vyšetření patří k základním metodám klinické genetiky. I přes značný rozvoj moderních metod molekulární genetiky jsou nepostradatelné při stanovení karyotypu a vyšetření vrozených chromosomových aberací, které se často podílí na vzniku vývojových vad, mentálních poruch nebo v dnešní době značně rozšířené problematice neplodnosti v páru. Stále častěji nachází uplatnění také v posouzení mutagenních účinků některých chemických látek na organismus u osob z rizikových pracovišť. Kultivace periferní krve je základním bodem cytogenetického vyšetření. Můžeme říci, že správný průběh kultivace určuje úspěšný průběh celého vyšetření. Cílem bakalářské práce bylo analyzovat dostupná data o provedených vyšetřeních, zejména u pacientů s neúspěšnou kultivací periferní krve a pokusit se vytipovat kritická místa, která by mohla mít vliv na průběh kultivace. Předpokládanými faktory byly věk pacienta, probíhající léčba a osobní anamnéza. Zejména přítomnost vrozených chromosomových aberací. Data byla zpracována pomocí základních statistických metod a byla hodnocena za období 2013 – 2014. Neúspěšné kultivace v Laboratoři klinické genetiky OLG FN Brno činily v roce 2013 z celkového množství 1013 požadovaných vyšetření 5 %. V roce 2014 vzrostl počet požadovaných vyšetření o 152 žádostí, z toho k opakovanému odběru bylo pozváno 4,7 % pacientů. Z celkového počtu 2185 požadovaných vyšetření v letech 2013 a 2014 došlo k 106 neúspěšným kultivacím. Nejpočetnější skupinu vyšetřovaných pacientů tvoří starší děti, dorost a dospělí. Druhou nejčetnější skupinou pacientů jsou novorozenci. Novorozenci také tvořili skupinu s nejvyšší mírou neúspěšné kultivace. V roce 2013 selhala kultivace u 24 % pacientů a v roce 2014 bylo přezváno k opakovanému odběru 23 % novorozenců. Farmakologická léčba se podílela na 17 % případů neúspěšné kultivace periferní krve, z toho ve 12 % případů se jednalo o léčbu antibiotiky. Vliv výskytu chromosomových aberací na neúspěšnost kultivace se nepodařila průkazně prokázat. Analyzovaný soubor pacientů byl příliš malý, aby bylo možné provést přesnější hodnocení. Srovnáním výsledných hodnot výskytu neúspěšné kultivace v jednotlivých letech s tabulkovou hodnotou Seznamu zdravotních výkonů s bodovými hodnotami, která zohledňuje 10 % kultur bez mitóz, můžeme říci, že jsou dodrženy stanovené limitní požadavky na metodu. Srovnáním hodnoty s doporučením evropské cytogenetické asociace, ovšem, která uvádí jako klíčovou hodnotu úspěšnosti 98 %, bylo by vhodné pokusit se vytipovat kritická místa, která negativně ovlivňují následný proces kultivace.

Abstract:

Cytogenetic testing is one of the basic methods of clinical genetics. Despite the substantial development of modern methods of molecular genetics are indispensable for karyotyping and examination of congenital chromosomal aberrations that often contributes to developmental defects, mental disorders or nowadays widespread problems of infertility in the couple. Increasingly, it also finds in assessing mutagenic effects of certain chemicals in the body of people which are working on high risk places. Cultivation of peripheral blood is a fundamental point of cytogenetic examination. We can say that the correct course of cultivation determines the successful conduct of the entire cytogenetic investigation. The aim of the thesis was to analyze the available data on performed examinations, especially in patients with failed culturing peripheral blood and through in-depth analyzes of cards of patients who experienced low increase in assessable mitosis or for which no increase to try to identify the critical points that could affect the course of cultivation. Determinable factors were the age of the patient, ongoing therapy and personal history. In particular the presence of congenital chromosomal aberrations. The data were processed using basic statistical methods and were evaluated for 2013 - 2014. Unsuccessful cultivation in the Laboratory of Clinical Genetics OLG FN Brno in 2013 amounted to a total amount of 1013 examinations required 5%. In 2014, the number of required examination of 152 applications, of which the repeated sampling were invited to 4.7% of patients. Of the 2185 required examinations in 2013 and 2014 were 106 failed cultivations. The most numerous group of patients examined consists of older children, adolescents and adults. The second largest group of patients are newborns. Newborns also formed a group with the highest rate of unsuccessful cultivation in 2013 failed cultivation in 24% of patients, and in 2014 was to re-subscribe 23% of newborns. Pharmacological treatment was involved in 17% of cases failed culturing peripheral blood, of which in 12% of cases of antibiotic treatment. Influence the incidence of chromosomal aberrations failure cultivation failed to prove conclusively. The analyzed group of patients was too small to allow for more accurate Score. The obtained results can however influence of drugs expected, especially the influence of their mutual interaction. By comparing the resulting total value unsuccessful cultivation in different years with the tabulated value list of medical procedures with point values, which reflects 10% of cultures without mitosis we may say that they are complied with prescribed limit demands on the method. If we compare the values with the recommendation of the European Cytogenetic Association, which sets as a key success rate of 98% it would be useful to try to identify critical areas that negatively affect subsequent cultivation process. These include factors which can affect the correct sampling, transport and subsequent processing of the peripheral blood to achieve better results.
