Radiogrequent ablation - the element of protocol of care about the patients with venous ulcer

Abstract:

Předkládaná práce se zabývá možností léčby žilní ulcerace pomocí endovaskulárních technik. V první části je standardizace RFITT metody na základě ex-vivo experimentu na telecích končetinách. Druhá část - RFITT metoda je prověřena na základě klinických dat z intervencí na 1186 končetinách pacientů ve všech klinických stádiích CVI. Výsledky prokázali, že RFITT je spojena se signifikantně lepším efektem, pokud je na žilní stěnu aplikována energie nad 35J/cm. Třetí část srovnává výsledky od 78 pacientů s RFITT a 85 pacientů se sono navigovanou pěnovou skleroterapií v klinickém stádiu C6 žilní nedostatečnosti. Obě metody vedly k urychlení hojení bércové ulcerace. Efektivita RFITT byla 88,7% a 89,4% u pěnové skleroterapie, v tomto bodě nebyl nalezen statisticky významný rozdíl. RFITT je spojena s nižším rizikem rekanalizace, p=0,0036. U pěnové skleroterapie byl nalezen významný vliv etiologie CVI jako rizikový faktor pro rekanalizaci, použitím X2 testu bylo p=0,0325. Obě metody se ukázali jako rychlé a efektivní.

Abstract:

The study aims to standardize the RFITT method and to compare the effectiveness of RFITT and ultrasound guided foam sclerotherapy in the reflux elimination with patients with clinical stage C6 of CVI. There is the ex-vivo experimental standardization of RFITT method on the calf legs in the first part. RFITT is examinated based on human interventions on 1186 legs in the second part. . The energy above 35J/cm is accociated with significant better results. The third - 78 patients with RFITT were compared with 85 patients with ultrasound guided foam sclerotherapy in our study. All of them were in clinical stage C6 of CVI. Both the methods let to acceleration of the wound healing period. The effectiveness of RFITT was 88,7% in this case and 89,4% in the group with sclerotherapy, there was no significant difference between both groups . The RFITT is associated with the lower risk of recanalization, p=0, 0036. The influence of the etiology was observed in the group with sclerotherapy as the significant risk factor for recanalization. Comparing the primary and secondary CVI using the X2 test p=0,0325. Both the methods proved to be fast and effective.
