Evaluation of the cervical cancer burden of the Czech population

Abstract:

V této diplomové práci se věnujeme karcinomu děložního hrdla, konkrétně zátěži české populace tímto karcinomem. Jedná se o zhoubné nádorové onemocnění, které je čtvrtým nejčastějším onemocněním u žen na celém světě, pokud nepočítáme kožní nádory. Cílem této diplomové práce je v praktické části zhodnotit časový trend zátěže karcinomu děložního hrdla u českých pacientek a následně diskutovat vliv preventivních vyšetření. První část práce je teoretická. V první kapitole jsou popsány základní epidemiologické ukazatele. Druhá kapitola se věnuje karcinomu děložního hrdla. Ve třetí kapitole je popsána prevence karcinomu děložního hrdla. Poslední teoretickou kapitolou jsou biostatistické metody, které se využívají na modelování onkologické zátěže. Jmenovitě se jedná o age-period-cohort modely, joinpoint regresi a Poissonovu regresi. Následně je představen Národní onkologický registr, jsou popsána data a konkrétní metody analýzy dat a poté jsou představeny výsledky. V praktické části práce je využita metoda joinpoint regrese a age-period-cohort (APC) modely. Pomocí joinpoint regrese bylo prokázáno, že v posledních letech trend incidence i mortality karcinomu děložního hrdla klesá. Na tento pokles má s velkou pravděpodobností vliv preventivních vyšetření, která v České republice probíhají od roku 1966. Dále bylo pomocí APC modelů zjištěno, že riziko onemocnění i riziko úmrtí na toto onemocnění je významně závislé na věku při onemocnění (úmrtí), periodě onemocnění (úmrtí) a kohortě narození. Vzhledem ke zvyšujícímu se pokrytí organizovaným screeningem a s tím spojeným nárůstem karcinomu in situ lze předpokládat další významné snižování incidence i mortality karcinomu děložního hrdla.

Abstract:

This diploma thesis deals with cervical carcinoma, namely the burden of the Czech population of this carcinoma. Worldwide, cervical cancer is the fourth most frequent cancer in women, excluding non-melanoma skin cancers. The aim of this diploma thesis is to evaluate the time trend of cervical cancer burden in Czech population and to discuss the influence of preventive examinations. The first part of this thesis is theoretical. The first chapter describes basic epidemiological indicators. The second chapter deals with cervical cancer. The third chapter describes the prevention of cervical cancer. In the last theoretical chapter biostatistical methods which are used for modeling the cancer burden are described, explicitly age-period-cohort (APC) models, joinpoint regression and Poisson regression. Subsequently, the National Cancer Registry is presented, data and specific data analysis methods are described and results are presented. In the practical part of the thesis the method of joinpoint regression and age-period-cohort models is used. Using joinpoint regression, it has been shown that in recent years the incidence and mortality rate of cervical cancer has been decreasing. The decrease of these rates in the Czech Republic since 1966 is likely to be influenced by the impact of preventive examinations. Using APC models it was also found that the risk of the disease and the risk of death from the disease is significantly dependent on the age of the disease (death), the period of disease (death) and cohort of birth. The significant reduction in incidence and mortality can be assumed from the increasing coverage of organized screening and the associated increase in the carcinoma in situ.
