Abstract:

Jedním z ústředních témat nádorové biologie je snaha o pochopení nekódujících RNA, zejména microRNA (miRNA), v patogenezi onkologických onemocnění. MiRNA jsou 19-25 nukleotidů dlouhé evolučně zakonzervované regulátory genové exprese, které v buňce kontrolují na posttranskripční úrovni jak řadu fyziologických, tak patologických procesů. Onemocněním, u nějž byla v roce 2002 poprvé pozorována souvislost s miRNA, byla chronická lymfocytární leukémie (CLL). Jedná se o nejčastější leukémii dospělého věku u západní populace a současně jednu z největších záhad mezi hematologickými malignitami díky extrémně variabilní prognóze a ne zcela objasněné patogenezi. Obecně bývá CLL popisována jako onemocnění v důsledku selhání apoptózy. Proto jsme se v práci zaměřili na identifikaci microRNA zapojených do apoptotické odpovědi a to zejména v souvislosti s její indukcí po terapii fludarabinem (F), který tvoří stěžejní část standardu terapie CLL. I přes dobrou odpověď na tuto léčbu většina pacientů relabuje a 10-15 % pacientů s agresivním subtypem je rezistentních. Proto bylo naším cílem identifikovat miRNA zapojené do odpovědi na F a pokusit se na základě jejich exprese identifikovat tyto pacienty. Na základě námi získaných dat jsme byli s využitím miR-34a, která byla nejvíce indukovanou miRNA, schopni odlišit pacienty s nejagresivnějším průběhem onemocnění a současně jsme identifikovali další miRNA potenciálně zapojené do odpovědi na F. Dalším krokem bylo vytvoření platformy pro klinickou aplikaci našich poznatků, která může být využita pro rychlý screening pacientů s progredujícím a potenciálně agresivním onemocněním

Abstract:

One of the main topics in cancer biology is the understanding of the role of microRNAs (miRNA) in pathogenesis of oncological diseases since the time the connection was established in 2002. MiRNAs are 19-25 nucleotides long conserved regions of non-coding RNAs. They serve as gene expression regulators for numerous physiological and pathological processes in the cell. The first connection made for miRNAs and malignancies was for chronic lymphocytic leukemia (CLL) and miR-15/16 cluster. CLL is the most common leukemia in adults in western world. It is also one of the biggest hematological mysteries because of its variable prognosis and not fully explained pathogenesis. CLL is usually described as an illness of failed apoptosis. For this reason we concentrated on miRNAs changed during the apoptotic response especially the ones induced by fludarabine (F), which is the medication of first choice in CLL. Even though the response to this type of therapy is quite good, patients would start to fail the therapy with 10-15% being resistant from the beginning. Our aim then was to identify the miRNAs connected to response to F and according to their expression identify such patients. We discovered 6 miRNAs involved in apoptotic response to F. At the basis of our data we were able to find the most induced miRNA, miR-34a and based on its expression we were able to identify the patients with more aggressive subtype of the disease. This encouraged us to the development of clinically applicable platform for absolute quantification of miR-34a. This platform can be used for quick screening for patients with progressive and potentially aggressive disease.
