Forensic estimation of therapeutical drug monitoring of citalopram, zolpidem and mirtazapine

Abstract:

Cílem práce bylo doplnit databázi intoxikací vybranými léčivy, které jsou na trhu po velmi krátkou dobu a ještě stále se prověřují jejich nežádoucí účinky, návykovost a toxicita. Byl shromážděn soubor 70 záchytů těchto léčiv kdy v 30 případech šlo o jednoznačnou intoxikaci. Z toho v 19 případech byly intoxikovány ženy. Letální intoxikace v monoterapii byla zaznamenána v 7 případech, v 1 případě citalopramem a v 6 se jednalo o intoxikaci zolpidemem. Byl sledován i záchyt alkoholu v uvedených případech. Dalším cílem práce bylo vytvořit a na co nejširším souboru reálných vzorků ověřit časově nenáročnou, dostatečně citlivou modifikaci metody stanovení vybraných léčiv. Jako tato metoda byla vybrána microextrakce s GC-MS koncovkou. Hladiny léčiv byly současně měřeny metodami standardně používanými a dobře popsanými v příslušné literatuře - kapalinovou chromatografií po extrakci do 200µl 0,05M kyseliny sírové a Solid Phase Extraction s koncovkou GC-MS. Dostatečná velikost naměřeného souboru je zde zárukou, že byly měřeny vzorky v různém stupni autolýzy. V závěru bylo konstatováno, že microetrakce s GC-MS koncovkou je vhodná metoda pro měření hladin vybraných léčiv v rozmezí terapeutických i toxických hladin.

Abstract:

The aim of this work was to complete the database of the cases of intoxication with selected remedies that are on the market for a very short time, and that are still investigated for their adverse effects, addiction and toxicity. There have been collected the batch of 70 cases of detection of these remedies, in 30 cases they were unambiguous intoxications; 19 cases were intoxications of women. There were diagnosed 7 cases of the lethal intoxication, they all were cases of intoxication by zolpidem. Within thementioned cases the blood alcohol levels were determined simultaneously. The another aim of this work was to create and verify on as wide file of the real samples as possible the time-sparing and sufficiently sensitive method of determination of selected remedies. As the mentioned method it was chosen microextraction coupled to GC-MS as an analytical method. The concentrations of the remedies were measured by standardly used methods and these are well described in appropriate literature – the liquid chromatography following the extraction to the 200µl 0.05M solution of sulphuric acid, and the solid phase extraction coupled to the GC-MS. The sufficient extent of the measured batch of real samples guarantee the possibility of analysis of the samples in different stage of autolysis. It is claimed in the conclusion that the method of microextraction coupled to GC-MS is suitable for the determination of selected remedy levels in the ranges of their therapeutical and toxic concentrations.
