Abstract:

Chirurgická léčba epilepsie je etablovanou metodou v léčbě pacientů trpících farmakorezistentní epilepsií. Důležitou součástí vyšetření pacientů je hodnocení EEG vyšetření, které může být hodnoceno jak vizuálně, tak s využitím pokročilého matematického zpracování. V první studii jsme se zaměřili na hodnocení signálu skalpového EEG a jeho vztah k efektivitě vagového stimulátoru (VNS). Zjistili jsme, že respondéři na VNS a nerespondéři se významně liší v několika typech entropie. V budoucnosti může být entropie použita při predikci účinnosti VNS před jeho implantací. Ve druhé studii jsme analyzovali invazivní EEG (iEEG), hodnotili jsme přítomnost interiktálních epileptiformních výbojů (IEDs), vysokofrekvenčních oscilací (HFO) a konektivity a jejich vztah k histopatologickému nálezu. Zjistili jsme, že charakteristiky těchto elektrofyziologických veličin korelují s histopatologickými nálezy. Poslední dvě studie analyzovaly korelaci mezi epilepsií a kvalitou života. V první z těchto studií jsme zjistili, že chirurgická léčba epilepsie ovlivňuje nejen frekvenci záchvatů, ale také kvalitu života. Druhá práce se zabývala dopadem pandemie COVID-19 na zátěž pacientů s epilepsií s doprovodným duševním postižením a jejich pečovatelů.

Abstract:

Epilepsy surgery is a well-established method in the treatment of epileptic patients. The critical part of drug-resistant epileptic patients’ examination is the evaluation of neurophysiology, which can be both, visual or with the employment of advanced EEG data post-processing. In the first study, we focused on assessing scalp EEG signal and its post-processing concerning VNS response regarding seizure reduction. We found that VNS responders and non-responders differ significantly in several entropy measures. In the future, entropy can be used in pre-implantation prediction of VNS efficacy. In the second study, we analysed invasive EEG (iEEG) in terms of the presence of interictal high-frequency oscillations (HFO), spikes and connectivity and their correlation with underlying pathology. We found that the characteristics of these electrophysiological variables reflect the histological finding. The last two studies analysed the correlation between epilepsy and quality of life. In the first of these studies, we found that epilepsy surgery did not influence only the seizure frequency, but also the quality of life. The second work was focused on the impact of the COVID-19 pandemic on the burden of epileptic patients with concurrent mental disability and their caregivers.
