Myocardial perfusion scintigraphy - pharmacological stress

Abstract:

Bakalářská práce se zabývá vyšetřením srdečního myokardu, konkrétně perfuzní scintigrafií myokardu, prováděnou na klinice Nukleární medicíny. Při vyšetření je nutné vyšetřovaného nejprve zatížit kvůli zvýšení srdeční činnosti, jež napomáhá v detekci možného defektu perfuze myokardu. Během zátěže se naaplikuje pacientovi kontrastní látka (radiofarmakum), umožňující zobrazení perfuze nahráváním obrazového záznamu pod gamakamerou. Při pozitivním nálezu je pacient zván na vyšetření v klidu. Práce se skládá z teoretické a praktické části. V teoretické práci je popsána anatomická stavba, funkce a zásobení srdce. Součástí jsou informace o nejběžnější chorobě prokrvení srdce. Dále pojednává o principech detekování radioaktivity, které dané vyšetření využívá. Poskytuje kompletní přehled o průběhu vyšetření i farmakologické zátěži a farmak, které ji způsobují. V praktické části je provedeno šetření 55 vybraných vyšetřovaných pomocí farmakologické zátěže za rok 2022 a 2023. Data byla získána na Klinice nukleární medicíny ve Fakultní nemocnici v Ostravě. Práce poskytuje grafický a tabulkový přehled zastoupení pacientů dle věku, pohlaví a průběhu vyšetření, sděluje informace o naměřených hodnotách krevního tlaku a tepové frekvence, udává výskyt vedlejších účinků po farmakologické zátěži.

Abstract:

The bachelor thesis deals with the examination of the myocardial perfusion, specifically myocardial perfusion scintigraphy, provided at the clinic of Nuclear Medicine. At first, the patient needs to be stressed to increase cardiac activity, which help with the detection of possible defect. During the stress is applied contrast agent ( radiofarmaceutical) which provides imagining data by recording by gamma camera. In case of a defect discover, the patient is invited for a examination in rest. The thesis consists of theoretical and practical parts. The theoretical part describes the anatomical structure, function, and blood supply of the heart. It includes information about the most common heart perfusion disease. It discusses the principles of detection of radioactivity used in the examination, providing a complete overview of the examination process and pharmacological stress testing incuding pharmaceuticals which iniciates stress. In the practical part, the examination of 55 selected patients using pharmacological stress was conducted for the years 2022 and 2023. The data were obtained at the Clinic of Nuclear Medicine of the University Hospital Ostrava. The thesis provides a graphical and tabular representation of patient distribution by age, gender, and examination process. The part includes information about the measured values of blood pressure and heart rate, and reports the occurrence of side effects related to pharmacological stress factors.
