Molecular identification of streptococci in patients with infective endocarditis

Abstract:

Infekční endokarditida (IE) je vzácné, život ohrožující infekční onemocnění endokardu. Ačkoliv jsou streptokoky druhou nejčastější příčinou IE, standardní metody druhové identifikace streptokoků a jejich nutričních variant nejsou dostačující. Výskyt streptokokových IE může být navíc v důsledku antibiotické terapie standardními kultivačními metodami podhodnocen. Spektrum deklarovaných druhů streptokoků způsobujících IE v České republice proto může být zkreslené. Vhodnou alternativou stanovení etiologie IE se jeví molekulárně-biologické metody, které umožňují přímý záchyt DNA patogenního agens bez nutnosti jeho kultivace. Cílem této diplomové práce bylo nalezení vhodné variabilní oblasti genu 16S rDNA, která by umožnila druhové rozlišení klinicky významných streptokoků. V práci byly porovnány sekvence tohoto genu u 32 druhů streptokoků a jim příbuzných bakterií. Jako nejvhodnější oblasti pro druhové rozlišení byly prokázány variabilní oblasti V2-V6. Vybrané oblasti byly následně využity při sekvenační analýze klinických vzorků pacientů s podezřením na streptokokovou IE, za účelem stanovení druhového zastoupení streptokoků. Nejčastěji byly identifikovány viridující streptokoky (56 %), především skupiny S. mitis a S. sanguinis, a streptokoky skupiny S. bovis (13 %). Navíc byly srovnány výsledky PCR a kultivačních metod. Výsledky hemokultur a kultivace z chirurgického materiálu se shodovaly s PCR v 53 % a 24 % případů. Tyto výsledky potvrdily význam PCR pro bližší identifikaci streptokoků a také pro detekci agens u kultivačně negativních případů, s čímž souvisí i lepší zmapování epidemiologie IE.

Abstract:

Infective endocarditis (IE) is an uncommon, life-threatening infective disease of the endocardium. Although streptococci are among the most common causes of IE, standard methods of species identification of streptococci and their nutritional variants are not sufficient. The incidence of streptococcal IE may also be underestimated due to false negative results of culture tests in patients under antibiotic therapy. Therefore the epidemiology of streptococcal IE in the Czech Republic may be inaccurate/incomplete. PCR-based methods seem a suitable alternative to determine the etiology of IE. The aim of the study was to find a variable region of 16S rDNA, suitable for distinguishing streptococci to species level. In this study, 32 16S rDNA sequences of reference streptococcal strains were compared. The most suitable regions for their determination were variable regions V2-V6. The chosen regions were used to assess the species representation of streptococci in patients with suspected streptococcal IE. Viridans streptococci (56 %), mainly S. mitis group and S. sanguinis group, and S. bovis group (13 %) were identified most frequently. In addition, results of PCR and standard culture were compared. Blood culture and tissue culture agreed with PCR in 53 % and 24 % of IE cases, respectively. We showed that PCR is a usefull tool for identification of streptococci especially in culture-negative cases enabling better undestanding on IE epidemiology.
