Nursing process in a pregnant woman with multiple pregnancies

Abstract:

Bakalářská práce řeší problematiku vícečetného těhotenství. Lékař považuje vícečetné těhotenství za rizikový stav, kde se může vyskytnout mnoho komplikací, které mohou ohrožovat život matky tak i plodů. Proto je v porodnictví toto těhotenství velmi hlídáno. Je proto velmi důležité znát informace od projevů až k léčbě. Bakalářská práce se soustředí na ošetřovatelský proces u těhotné ženy s vícečetným těhotenstvím. Bakalářská práce je rozdělena na teoretickou a praktickou část. Cílem práce je sestavení ošetřovatelského plánu, který by mohly využívat porodní asistentky v praxi. Teoretická část se zabývá ošetřovatelským procesem, vznikem vícečetného těhotenství, výskytem a typy plodů. Tato část je zaměřená na postup v péči o těhotnou ženu s vícečetným těhotenstvím. Jsou zde vypsána pravidelná i nepravidelná vyšetření, které žena postupuje během těhotenství a změny těla. V bakalářské práci jsou i vypsané komplikace, které mohou nastat během těhotenství. Praktická část práce se zabývá anamnézou klientky a průběhem hospitalizace na porodním sále a rizikovém oddělení. Díky získaným informacím ze zdravotní dokumentace, laboratorních vyšetření, provedených fyzikálních vyšetření porodní asistentkou a informacemi od určené pacientky byl sestaven ošetřovatelský proces v jednotlivých doménách podle NANDA I. taxonomie II. Tyto domény se vztahují na určité potřeby pacientky s vícečetným těhotenstvím. V ošetřovatelském procesu byly stanoveny ošetřovatelské cíle, intervence, realizace a závěrečné hodnocení.

Abstract:

The bachelor thesis addresses the issue of multiple pregnancy. The doctor considers multiple pregnancy as a risky condition where many complications can occur that can threaten the life of both the mother and the fetus. Therefore, in obstetrics, this pregnancy is highly guarded. It is therefore very important to know the information from manifestations to treatment. This bachelor thesis focuses on the nursing process of a pregnant woman with multiple pregnancies. The bachelor thesis is divided into theoretical and practical part. The aim of the thesis is to develop a nursing plan that could be used by midwives in practice. The theoretical part deals with the nursing process, the occurrence of multiple pregnancies, the incidence and types of fetuses. This section focuses on the process of caring for a pregnant woman with a multiple pregnancy. The routine and non-routine examinations that a woman undergoes during pregnancy and body changes are listed. The complications that may occur during pregnancy are also listed in the bachelor thesis. The practical part of the thesis deals with the history of the client and the course of hospitalization in the delivery room and the risk ward. With the information obtained from the medical records, laboratory tests, physical examinations performed by the midwife and information from the identified patient, the nursing process in each domain was constructed according to NANDA Taxonomy I Taxonomy II. These domains relate to specific needs of the patient with multiple pregnancies. The nursing process established nursing goals, interventions, implementation and final assessment.
