Postnatal quality of life of women after normal vaginal delivery and caesarean section

Abstract:

Cíl: Hlavním cílem práce bylo zjistit kvalitu života žen po spontánním porodu a po císařském řezu. Dále zjistit, zda se hodnocení kvality života žen po porodu liší v závislosti na způsobu porodu, věku, vzdělání, počtu porodů, rodinném stavu, pohlaví dítěte a cvičení v šestinedělí. Metodika: Výzkumný soubor tvořilo 136 žen, které po období šestinedělí navštívily vybrané gynekologické ambulance v Ostravě od května do října 2018 a zároveň splňovaly kritéria pro zařazení do souboru. Přičemž 100 žen porodilo spontánně a 36 žen císařským řezem. Pro sběr dat byl použit standardizovaný dotazník SF-36, kterému předcházely demografické a doplňující otázky. Výsledky: Výsledky nevypovídají o jednoznačně rozdílné kvalitě života žen v závislosti na způsobu porodu. Ovšem byly zjištěny rozdíly u sociálního fungování a fyzické bolesti, které poukazují na nižší kvalitu života žen po císařském řezu. Další testování zjistilo vyšší kvalitu života žen ve skupině 30-34 let u domény fyzické omezení i fungování. U vitality žen byla zjištěna vyšší kvalita života žen s vyšší odbornou a vysokou školou, a naopak nejnižší kvalitu měly ženy se základním vzděláním. I v duševním zdraví opět hodnotily svou kvalitu života jako nižší, ženy se základním vzděláním. U ostatních cílů práce další významné rozdíly shledány nebyly. Závěr: Těhotenství a porod je běžný fyziologický jev, přesto však u mnoha žen zanechává trvalé následky, které mohou ovlivnit pocit zdraví a kvalitu života. Výsledky výzkumu ukazují na prožívání a potřeby žen během poporodního období.

Abstract:

Objective: The aim of this thesis was to find out the quality of life of women after spontaneous delivery and caesarean section. Next aim included finding out the differences between evaluation of quality of life of women after delivery in dependence on their way of giving birth, age, education, number of births, marital status, child's sex and exercise in the puerperium. Methods: The research sample composed of 136 women after childbirth who visited chosen gynaecologist ambulances after the puerperium period in Ostrava from May to October 2018 and those who satisfied the conditions for submission in the sample. In the sample 100 women delivered their child spontaneously and 36 women underwent caesarean section. Data was collected by using the standardized questionaire SF-36, together with demographic and supplementary questions. Result: Results did not show any significant differences between quality of life of women in dependence on the way of birth. On the other hand the differences were find out in social functioning and physical pain. Women that underwent the caesarian section alleged about lower quality of life. In the domain of physical limitation and functioning the women at the age of 30-34 years showed higher quality of life. Women with higher education (included vocational education and college education) reported higher quality of life compared to women with education on elementary school. These women with elementary education also evaluated their quality of life as lower, in the domain of sanity. In the rest of this thesis there were not find out any significant differences. Conclusion: Pregnancy and childbirth is normal physiological phenomenom, but they can leave permanent consequences for many women, which may affect their health and quality of life. Research results indicates the needs and experiences of women during the postpartum period.
