Stanovení kreatininu v séru u vybraných skupin pacientů pomocí spektrofotometrické (Jaffé), enzymatické a HPLC metody

Abstract:

Kreatinin je dusíkatá látka nebílkovinné povahy. Vzniká jako produkt přeměny kreatinfosfátu při energetickém metabolismu svalů. Jako odpadní látka je vylučován močí. Hladina kreatininu v séru se nazývá kreatininémie. Této hodnoty lze využít k výpočtu clearence kreatininu, tedy k odhadu glomerulární filtrace. Ta udává stupeň poškození ledvin. Clearence lze vypočítat díky znalosti koncentrace kreatininu v séru ze vzorce dle Cockrofta a Gaulta. Zvýšené hladiny kreatininu tedy poukazují na poškození ledvin. Jsou popisovány i korelace mezi vysokou kreatininémií a úmrtím na kardiovaskulární choroby. Jako nejčastější příčina sekundárního poškození ledvin je udáván diabetes mellitus. U analogů kreatininu jako je cyklokreatin však byly popsány účinky protinádorové, antivirové, antidiabetické, ale i ochranné před neurodegenerativními nebo svalovými postiženími. Pro stanovení kreatininu v séru byla použita metoda HPLC s UV detekcí. Byla použita analytická kolona MAG1 4,6 x 250 mm, Labiospher PSI 100 C18, 5 ?m, opatřena PEEK filtrem (ESA Inc.). Mobilní fáze byla 25 mmol/l NaH2PO4 ? ethanol (97:3, v/v), pH = 6,5. Enzymatická souprava byla od společnosti PLIVA-Lachema Diagnostika s r.o., Brno. Kalibrační křivka byla lineární v celém rozsahu koncentrací použitých standardů kreatininu. Analytické parametry byly vyhovující. Hladina kreatininu v séru pacientů jednotlivých souborů se pohybovala v těchto rozmezích: pacienti s diabetem 98,2 - 119,9 ?mol/l (medián 109,0 ?mol/l), pacienti s ikterem 67,5 - 118,3 ?mol/l (medián 92,9 ?mol/l), pacienti s kardiovaskulárním onemocněním 83,2 - 118,0 ?mol/l (medián 100,6 ?mol/l), pacienti s onemocněním ledvin 48,5 - 374,2 ?mol/l (medián 211, 4 ?mol/l) a kontrolní skupina pacientů s ostatními diagnózami 80,9 - 150,8 ?mol/l (medián 117,9 ?mol/l).

Abstract:

Creatinine is a nitrogenous substance of non-proteinaceous nature. It is a product of the conversion of creatine phosphate within the process of energy metabolism in muscles. As a waste substance it is excreted in the urine. The level of creatinine in serum is called creatininemia. This value can be used to calculate creatinine clearance, therefore, to estimate glomerular filtration rate. This indicates the degree of kidney damage. Clearance can be calculated through knowledge of creatinine level in serum and the formula according to Cockroft and Gault. Elevated levels of creatinine, thus, indicate kidney damage. There is also a described correlation between high creatininemia and death caused by cardiovascular disease. Diabetes mellitus is given as the most frequent cause of the secondary damage of kidney. At the analogues of creatinine, i.e. cyclocreatine, the anti-cancer, antiviral, antidiabetic and also protective effects against neurodegenerative or muscular disabilities were described. For the determination of serum creatinine HPLC with UV detection was used. Analytical column was used MAG1 4.6 x 250 mm Labiospher PSI 100 C18, 5 mm, fitted with PEEK filter (ESA Inc.). Mobile phase was 25 mmol / l NaH2PO4 - ethanol (97:3, v / v), pH = 6.5. The enzymatic kit was by PLIVA-Lachema Diagnostics Ltd., Brno. The calibration curve was linear over the entire range of concentrations of used creatinine standards. Analytical parameters were satisfactory. The levels of creatinine in serum of the individual patient files were in the following ranges: patients with diabetes from 98,2 to 119,9 ?mol/l (median 109,0 ?mol/l), patients with icterus from 67,5 to 118,3 ?mol/l (median 92,9 ?mol/l), patients with cardiovascular disease from 83,2 to 118,0 ?mol/l (median 100,6 ?mol/l), patients with disease of excretory system from 48,5 to 374,2 ?mol/l (median 211,4 ?mol/l) and control group of patients with other diagnoses from 80,9 to 150,8 ?mol/l (median 117,9 ?mol/l).
