Physiotherapy of enthesopathy of the elbow joint

Abstract:

Úvod: Pro vypracování mé bakalářské práce jsem si vybrala téma popisující fyzioterapii entezopatií loketního kloubu. Záměrem prostudování teoretických poznatků a seznámení s danou problematikou bylo přiblížit se optimalizaci zdravotního stavu probanda. Cíl práce: Cílem bakalářské práce je přispět k řešení problematiky fyzioterapie u pacienta s entezopatií loketního kloubu. Metodika: Hlavní část mé závěrečné práce tvoří kazuistika pacientky s laterální epikondylitidou. Spolupráce, kterou popisuji včetně vyšetření a terapií, probíhala zhruba 2 a půl měsíce po týdnu od sundání sádrové fixace loketního kloubu a zápěstí. V úvodu jsem vypracovala vstupní kineziologický rozbor, na jehož základě jsem určila krátkodobý fyzioterapeutický plán. V polovině mé terapeutické intervence jsem provedla kontrolní vyšetření. Náplní terapií se staly především techniky měkkých tkání, mobilizace, PNF, PIR, cvičení s/bez využití pomůcek v otevřeném či uzavřeném kinematickém řetězci, DNS a ACT, s cílem ovlivnění bolesti a hypertonického svalstva společně s jeho reflexními body, ošetření měkkých tkání a joint play, zvětšení rozsahu pohybu a svalové síly s aktivací hlubokého stabilizačního systému. Důraz jsem kladla na korekci ergonomie pracovního prostředí a využití Flossingu při terapii. Po dokončení rehabilitace jsem taktéž vypracovala výstupní kineziologický rozbor, s jehož pomocí jsem stanovila dlouhodobý fyzioterapeutický plán. Výsledky: Výsledkem mé práce byly viditelné změny v oblasti protažitelnosti a posunlivosti měkkých tkání, vymizení reflexních bolestivých bodů, vstřebání otoku, optimalizace joint play, snížení tonu většiny inkriminovaných svalů, ale především pokles míry bolestivosti. Ačkoliv se mi taktéž podařilo terapii zacílit na snížení rozsahu pohybu a svalovou sílu, v některých případech pravá horní končetina stále zaostává za tou levou. Závěry: V závěru práce jsem posuzovala výsledky vstupního a výstupního kineziologického rozboru. V závislosti na ně mohu konstatovat, že má terapeutická intervence měla na pacientku pozitivní vliv.

Abstract:

Introduction: For my bachelor´s thesis completion, I have chosen a theme, which closely describes physiotherapy of enthesopathy in elbow joint. It is main purpose being the introduction to the main issue. The objective: Main goal of this thesis is to contribute to better and further understanding and resolve of the issue of enthesopathy physiotherapy. Methodology: Main part of thesis consists of casuistry of patient with lateral epicondylitis. The collaboration I intend to describe, including the examination and therapy, took roughly two and half months, beginning week after taking the plaster off. First, I have worked on the entrance kinesiologic analysis, upon which I have constructed a short term rehabilitative plan. Further on, in the half point of the rahabilitation, I have conducted an examination of the progress my patient achieved. Therapy itself consisted mainly of techniques of soft tissue, mobilization, PNF, PIR, exercise with or without use of aids in open or close kinematic circuitry, DNS and ACT with goal being influence on pain and hypertonic muscles, including its reflective points, treatment of soft tissue and joint play, improvement of the movement area and muscle strength with activation of deep stabilitative system. I have taken great emphasis on correction of workplace ergonomics and use of Flossing during the therapy. I have also concluded the rehabilitation by conduction of an exit kinesiologic analysis with help of which I have constructed a long term rehabilitative plan. Results: My work resulted in visible shifts in the areas of extensibility and mobility of soft tissue, disappearance of reflective pain spots, swelling absorption, joint play optimisation, tone decrease in incriminated muscles, but most importantly - reduction of the extent of pain. Although, I've been successful in focusing on loss of muscle strenght and range of movement, in some cases, the right upper limb still falls behind the progress made by the other, left limb. Conclusions: In the conclusion to my work, I have also compared the results of entrance and exit kinesiologic analysis. Upon which I can state, that my therapeutical intervention have had a positive influence on my patient.
