Stereotactic Radiosurgery of Lung

Abstract:

Tématem mé bakalářské práce je stereotaktická radiochirurgie plic. Jedná se o léčbu pomocí CyberKnife systému, která je neinvazivní alternativou a využívá se u pacientů, kteří nemohou z jakéhokoliv důvodu podstoupit operaci. CyberKnife dodává do nádorů velmi přesné a vysoké dávky záření, zatímco šetří zdravou okolní tkáň. Jedinečnost CyberKnife systému je ve schopnosti bezpečně zaměřit plicní nádor v blízkosti kritických struktur i přesto, že pacient dýchá, a to s přesností až na 2 mm. Tato metoda léčby má výrazně méně komplikací, je bezbolestná, velmi přesná a bezpečná, nabízí pacientům větší šanci na vyléčení. V první kapitole teoretické části popisuju stereotaktickou radioterapii a radiochirurgii, zabývám se indikacemi, plánováním léčby a přístrojovým zastoupením. Další kapitola je zaměřena na samotný orgán plic. Začíná popisem anatomie a fyziologie, a postupně přechází ke karcinomu a popisu jejich diagnostice a léčbě. Ve třetí kapitole je zmíněna problematika účinku dávek záření na samotný orgán a jeho toleranční dávky. Čtvrtá kapitola objasňuje konvenční radioterapii, ozařovací objemy, frakcionační režimy a dávky. Poslední kapitola teoretické části popisuje už nadpisem zmiňovanou stereotaktickou radiochirurgii plic. V této kapitole se podrobněji zabývám samotným přístrojem CyberKnife a postupům při léčbě karcinomu plic na této modalitě. Studie je zaměřena na srovnání léčby primárního karcinomu plic na lineárním urychlovači a systému CyberKnife. Do studie bylo zahrnuto celkem 149 pacientů, ze kterých 115 pacientů podstoupilo léčbu na lineárním urychlovači a 34 pacientů podstoupilo léčbu systémem CyberKnife.

Abstract:

The theme of my bachelor?s diploma is stereotactic radiosurgery of lungs. It is medical treatment with help of CyberKnife system, which is non-invasive alternative used by patients that cannot from any reason undergo the operation. CyberKnife produces very precise and high doses of radiation directly into tumors without damage of surrounding healthy tissue. CyberKnife system is unique because it can safely locate lung tumor in the closeness of critical structures even though the patient breathes with the accuracy of 2 mm. This treatment method has distinctively fewer complications, is painless, extremely precise and safe and provides patients higher chance of recovery. In the first chapter of the theoretical part I describe stereotactic radiotherapy and radiosurgery; I deal with indications, planning of treatment and device representation. The next chapter is focused on the lung organ alone. It starts with the description of anatomy and physiology and gradually proceeds to carcinoma and to its diagnosis and treatment. The third chapter mentions problems with the effect of the radiation dose on the organ itself and its tolerance dose. Chapter number five explains conventional radiotherapy, radiation volumes, fractionating modes and dosage. The last chapter, already mentioned in the heading, is about stereotactic radiosurgery of lungs. Here I explore into detail the CyberKnife system alone and its techniques in the treatment of lung cancer on this modality. The study is focused on the confrontation of the primary lung cancer treatment with help of linear accelerator and the CyberKnife system. 149 patients were included into this study of which 115 patients underwent the linear accelerator treatment and the other 34 patients experienced treatment with CyberKnife system.
