Mass casualities - model techniques and resolutions in preehospital care

Abstract:

Mimořádné události s hromadným postižením zdraví jsou stále častější realitou dnešních dní. Množí se případy dopravních a průmyslových havárií, přírodních katastrof, ale v úvahu přicházejí i teroristické útoky. Zkvalitnění záchranných prací prováděných Zdravotnickou záchrannou službou (ZZS) a ostatními složkami Integrovaného záchranné systému (IZS) v přednemocniční etapě vyžaduje jednotný, kvalitně zpracovaný, doporučený postup odpovídající situaci a podmínkám v České republice (ČR). Na základě statistických výstupů této studie a s ohledem na zahraniční zkušenosti jsem vypracoval základní modelový postup zásahu ZZS a jeho varianty pro nejčastěji se vyskytující typy HPZ v ČR. Součástí práce je též několik tabulek umožňujících plánovat potřebný počet sil a prostředků, včetně materiálního a technického vybavení, pro řešení konkrétního typu a rozsahu MU. Předpokládaný jednotný postupu ZZS umožnil také definovat potřebný rozsah a způsob součinnosti ze zbylými základními složkami IZS.

Abstract:

Emergency situations with mass casualties represent a common reality nowadays. Traffic and industrial accidents or natural disasters occur more and more frequently, but terrorist attacks come to question, too. Improving the quality of rescue efforts, performed by Emergency Medical Services (EMS) and other parts of the Integrated Rescue System during the pre-hospital phase, requires an unified, considered, recommended method appropriate to the situation and conditions in the Czech Republic. Results of this study could be a base for the definitive form of such recommended method. Data of all documented interventions of Emergency Medical Services in the Czech Republic were evaluated in cases with higher number of patients (over 10) during last 10 years, it is 26 actions in total. Following parameters were evaluated in all these mass accidents: used staff and resources, types of emergency situation, number of casualties, types of injury, time intervals, evaluation method of health state severity and method of triage, treatment techniques and material consumption, distribution of patients to health care facilities, co-operation within the Integrated Rescue System and other aspects of the action. Based on statistical results of this study and with regard to experiences from abroad, the basic model method for the EMS intervention was worked out and its variants for the most frequently occurring types of emergency situations in the Czech Republic. A part of the study are also some tables enabling planning of the necessary number of staff and resources, incl. the material and technical equipment, to solve an emergency situation of a specific type and extent. The supposed unified method of EMS intervention enabled to define the necessary extent and method of interoperability with other basic parts of the Integrated Rescue System, too.
