Cévní protéza s distální bifurkací jako možná prevence tvorby intimální hyperplázie - experimentální studie na animálním modelu

The new vascular graft device with the distal bifurcation as a possible prevention for the intimal hyperplasia development - an experimental animal study

Abstract:

Dlouhodobá průchodnost syntetických femoro-popliteálních bypasů zůstává poměrně nízká. Abnormální hemodynamické poměry distální anastomózy a jimi vyvolané změny ve smykovém stěnovém napětí jsou považovány za hlavní příčiny selhání anastomózy. Výsledkem numerických a počítačových modelů byl návrh nového typu protézy s distální bifurkací. Naší hypotézou bylo, že tento nový typ protézy bude eliminovat nebo významně redukovat rozvoj intimální hyperplázie v oblasti anastomózy, a v budoucnu by tak mohl poskytnout alternativní přístup k revaskularizačním výkonům na ischemických končetinách. Účelem práce bylo ověřit naši hypotézu na animálním modelu a stanovit tak případný klinický potenciál nově navržené protézy. Nový typ anastomózy jsme srovnávali s tradiční end-to-side anastomózou, přičemž hlavním srovnávacím kritériem byl stupeň tvorby intimální hyperplázie. Dále bylo naší snahou demonstrovat její chirurgickou použitelnost, respektive implantabilitu. Použili jsme prasečí model, šesti prasatům jsme implantovali nový typ protézy, a dalším šesti prasatům tradiční protézu bez větvení na konci. Všechna zvířat byla po 20 týdnech usmrcena, a poté byl explantát protézy s koncovými úseky nativních tepen podroben histologické a morfometrické analýze. Rozdíly v průchodnosti ve stanovených profilech byly mezi oběma typy protéz hodnoceny pomocí mediánového testu. Výsledky studie nedávají jasný důkaz, že by navrhovaný typ protézy měl klinické aplikace a díky menší tvorbě intimální hyperplazie zlepšoval průchodnost, a to navzdory tomu, že in vitro metody ukazovaly na jasný hemodynamický benefit.

Abstract:

The long-term patency rates of synthetic femoral bypass grafts remain poor. Abnormal hemodynamics and changes in wall shear stress are commonly agreed to be a major contributors to the development of disease at the junction. The computational and experimental studies propose a new vascular graft consisting of a normal straight bypass graft with a new distal bifurcation added to it. Our hypothesis was, that this new device leads to significant decrease in development of intimal hyperplasia at the junction. The purpose of the study was to introduce this novel design, to demonstrate its surgical feasibility and compare it with the traditional distal end-to-side anastomosis. We have used porcine model, six of the animals were implanted with a new graft device and six animals had a conventional straight vascular graft. Each pig was destroyed 20 weeks after surgery, the device together with end sections of the aorta and iliac artery was excised for histological assessment and morphometric analysis. Patency rates differences between profiles in experimental and control group were tested using the median test. Although in vitro methods have shown significant hemodynamic benefits, the result of the study did not provide sufficient evidence that the proposed device has in vivo applications and improves patency rates by means of decreasing the intimal hyperplasia development.
