Compliance to correct procedure in the application of transfusion (creation of performance standard for nursing)

Abstract:

V této bakalářské práci se zabývám zejména postupem při podávání krevní transfuze, dodržováním předepsaného standardu a sledováním chyb při přípravě a podávání krevní transfuze. Vybrala jsem si dva soubory, které zkoumám. Tyto dva soubory zahrnují interní a chirurgickou jednotku intenzivní péče nemocnice krajského typu. Teoretická část je zaměřena na fyziologii a jednotlivé složky krve, krevní skupiny a Rh faktor, krevní transfuzi, která obsahuje něco málo z historie, darování krve, transfuzní přípravky, jejich skladování a manipulace, druhy transfuzí, kompetentní osoby, potransfuzní reakce. Nejvíce je soustředěna na povinnosti sestry před a po aplikaci krevní transfuze, techniku provedení transfuze, kontrolu krevní skupiny u lůžka, biologickou zkoušku, péči o nemocného během transfuze. Jedním z cílů této bakalářské práce je vytvoření standardu, proto se i v teorii objevují základy kvality ošetřovatelské péče. Vysvětlení pojmu standard, jednotlivé typy standardů, tvorba standardu a audit. Praktická část sděluje výsledky pozorování při podávání krevní transfuze. Zejména v oblasti přípravy k provedení transfuze, informovanosti a poučení pacienta, vlastní transfuze a ukončení transfuze. Cíle a výzkumné otázky jsou vyhodnoceny v diskuzi. Porovnání výsledků jednotlivých zkoumaných souborů je uvedeno v závěru. Dále obsahuje doporučení pro praxi ve formě vypracovaného standardu.

Abstract:

This bachelor thesis deals with the procedure of giving blood transfusion, respecting specified standards and monitoring mistakes during the preparations and application the blood transfusion. I have choosen two complex which i research. These two complex involve internal and surgical intensive care unit of a regional type. Theoretical part is focusing on physiology and blood components, blood types and RH factor, blood trasfusion which involve a little from history, blood donation, transfusion products and their storage and manipulation with them, types of transfusion, persons with competents, after transfusion reaction. Most concentrated is on the duties of nurse before and after aplication of transfusion, their technique of giving transfusion, checking blood types at the hospital bed, biological test and taking care about the patient during the transfusion. One of the objectives in this bachelor thesis is create a standard, because of this the theory involve basics of quality of nursing care - explanation the term of standard, different types of standards, how to create standards and audit. The practical part informate us about the results in observation during the aplication of blood transfusion. Especially in the part of preparation for aplication the blood transfusion, information and education patient about the transfusion, beggining and finishing blood transfusion. Objectives and research questions are evaluated in the discussion. Comparison of results of each research complex are shown in the end of this study. Next include recommendations for practice in the form of prepared standard.
