Breast cancer- Preventive care in asymptomatic individuals from high-risk cancer families and patients with genetic predisposition.

Abstract:

Karcinom prsu je zhoubné onemocnění prsní tkáně, které tvoří 16% všech maligních onemocnění u žen v České republice. Patří tedy mezi nejčastější nádorové onemocnění a v současnosti jej lze považovat za jeden z nejzávažnějších epidemiologických problémů. Cílem diplomové práce je určit či zpřesnit možnosti preventivní péče a zdravotní rizika u osob, kterým byla na základě molekulárně genetické analýzy potvrzena mutace v genu CHEK2, který se podobně jako geny BRCA1/2 účastní procesu oprav poškozené DNA. K molekulárně genetické analýze byl použit soubor 109 probandů, kteří byli indikováni klinickými genetiky v rámci celé České republiky. U sedmi z nich byla potvrzena pozitivita v genu CHEK2, u kterého byly sledovány dva nejčastější typy mutací. Tyto osoby byly zařazeny do skupiny žen s vysokým rizikem vzniku ca prsu (nad 20%). U zbylých 102 pacientů nebyla mutace v genu CHEK2 potvrzena, to však neznamená, že proband je negativní, protože může mít jakoukoliv jinou sekvenční variantu v jiném z minoritních (nízce penetrantních- např. PALB2) genů, či v jiném typu mutace v genu CHEK2, které se dosud v Laboratoři AGEL a.s. nevyšetřuje. Z této skupiny byly vybrány tři negativní probandky na ukázku výpočtu Clausova modelu, který sloužil k zařazení do určité skupiny žen s rizikem vzniku ca prsu a s tím spojené zpřesnění preventivní péče. Relativní četnost mutací v Laboratoři AGEL a.s. v genech BRCA1/2 byla zjištěna v 16,54% a mutace v genu CHEK2 v 6,40%. Veškeré poznatky získané během této studie byly a budou dále použity v rámci rutinní praxe v Laboratoři lékařské genetiky, Laboratoře AGEL a.s.

Abstract:

Breast cancer is a malignant disease of the breast tissue, which account for 16% of all malignancies in women in the Czech Republic. It belongs to the most frequent cancer and now it can be considered as one of the most serious epidemiological problems. The thesis aim is to identify or refine the possibilities of preventive care and health risks for people who have been analyzed by molecular genetic methods and are positive for mutation in the CHEK2 gene, which is as well as the BRCA1/2 genes are involved in repairment of damaged DNA. For molecular genetic analysis the cohort of 109 probands who were indicated by clinical genetics throughout the Czech Republic was used. Seven of them were positive for mutation in the CHEK2 gene, in which two most common types of mutations were analysed. These persons were included in the group of women with high risk for breast ca (over 20%). 102 remaining patients had no mutations in the gene CHEK2, this does not mean that the proband is negative, they may have other sequence variants in a minority genes (e.g. low-penetranting PALB2 gene) or another type of mutations in the CHEK2 gene, which are still not analysed in the laboratory AGEL a.s.. From this cohort, three negative probands were selected for calculation of Claus model, which is used for classification of women to certain groups with increased risk of breast ca and to refine preventive care. The relative frequency of mutations in the Laboratory AGEL a.s. was 16.54% in genes BRCA1/2 and 6.40% in CHEK2 gene All information obtained during this study will be used in routine practice of the Laboratory of Medical Genetics, Laboratory AGEL a.s.
