Abstract:

Úvod: Zajištění pravidelnosti a správnosti fyzioterapie v domácím prostředí je jedním ze základních předpokladů úspěšné léčby. Studie srovnává balanční terapii v domácím prostředí na běžné balanční podložce a na kulové úseči, která poskytuje terapii doplněnou o biofeedback a zároveň umožňuje sledování množství a kvality jejího používání. Cíl: Cílem studie bylo porovnat vliv používání standardní balanční pomůcky a balanční pomůcky s biofeedbackem na úspěšnost fyzioterapie prováděné v domácím prostředí. Metodika: Do studie bylo zařazeno 55 pacientů po úrazech kolenního kloubu. Kontrolní skupinu tvořilo 25 osob (12 žen a 13 mužů, věk 39 ? 12 let) a experimentální skupinu tvořilo 30 osob (19 žen a 11 mužů, věk 40 ? 12 let). Srovnání účinku léčby bylo provedeno pomocí stabilometrické plošiny. Studie byla zahájena v prosinci 2019 a ukončena v květnu 2021. Po úvodním vyšetření na stabilometrické plošině následovala domácí balanční terapie trvající 3 týdny. Kontrolní skupina cvičila na balanční podložce Airex a experimentální skupina prováděla terapii na kulové úseči umožňující vizualizaci cvičení. Doporučená frekvence cvičení pro obě skupiny byla 5krát týdně s dobou terapie přibližně 15 minut. Při výstupním vyšetření byly obě skupiny opět vyšetřeny na stabilometrické plošině a efekt rehabilitace mezi skupinami byl zhodnoceny statistickými testy na hladině významnosti 5 %. Výsledky: Testování na stabilometrické plošině ukázalo statisticky významné rozdíly změny vstupního a výstupního měření v parametrech, které představují dynamickou stabilitu pacientů. Statisticky významný rozdíl byl zjištěn pro parametry Tar_P (p < 0,001) a Purs_P (p = 0,010). P_Tot, který představuje celkovou hodnotu změřenou balanční platformou ICS, vykazoval statisticky signifikantní rozdíl na úrovni p = 0,019 pro experimentální skupinu ve srovnání s kontrolní skupinou. Studie neprokázala vliv různých metod komunikace s pacienty během domácí rehabilitace. Závěr: Používání balanční pomůcky s biofeedbackem pozitivně ovlivňuje výsledky fyzioterapie v domácím prostředí. Pacienti využívající tuto rehabilitační pomůcku dosahovali lepších výsledků v testech zaměřených na dynamické parametry posturální stability.

Abstract:

Introduction: Ensuring regularity and correctness of physiotherapy in the home environment is one of the basic prerequisites for successful treatment. This study compares balance therapy in the home environment on a conventional balance mat and on a wobble board, which provides therapy supplemented with biofeedback and also allows monitoring of the quantity and quality of its use. Aim: The aim of the study was to compare the effect of using a standard balance aid and a balance aid with biofeedback on the success of physiotherapy delivered in the home environment. Methods: 55 patients with knee injuries were included in the study. The control group consisted of 25 subjects (12 women and 13 men, aged 39 ? 12 years) and the experimental group consisted of 30 subjects (19 women and 11 men, age 40 ? 12 years). Comparison of the treatment effect was performed using a stabilometric platform. The study started in December 2019 and ended in May 2021. The initial examination on the stabilometric platform was followed by home balance therapy lasting 3 weeks. The control group exercised on an Airex balance mat and the experimental group performed therapy on a wobble board that allowed visualization of the exercise. The recommended frequency of exercise for both groups was 5 times a week with a therapy time of approximately 15 minutes. At the final examination, both groups were again examined on the stability platform and the effect of rehabilitation between the groups was assessed using statistical tests at the level of significance of 5%. Results: The stabilometric platform tests showed statistically significant differences in the change between the input and output measurements in the parameters that represent the dynamic stability of the patients. A statistically significant difference was found for the parameters Tar_P (p < 0.001) and Purs_P (p = 0.010). P_Tot, which represents the total value measured by the ICS balance platform, showed a statistically significant difference at the level of p = 0.019 for the experimental group compared to the control group. The study did not demonstrate the effect of different communication methods with patients during home rehabilitation. Conclusion: The use of balance aids with biofeedback positively influences the results of physiotherapy at home. Patients who used this rehabilitation aid performed better on tests focusing on dynamic postural stability parameters.
