Postoperative delirium in a geriatric patient in intensive care

Abstract:

Pooperační delirium představuje závažnou komplikaci ve všech věkových skupinách pacientů. Delirium vyskytující se u seniorů, kteří již mnohdy mají chronická onemocnění, je o to více závažné. Delirium způsobí akutní postižení mozku. Základní projevy jsou kognitivní poruchy, poruchy paměti, orientace, poruchy vnímání, halucinace atd. Tyto změny mohou vést mimo jiné k prodloužení délky hospitalizace. Odborná literatura a výzkumy se této problematice věnují, ale bohužel stále neexistuje spolehlivý návod, jak úplně zabránit rozvoji pooperačního deliria. Teoretická část práce ve své první části popisuje možnosti anestezie a také to, že anestezie je pro geriatrického pacienta mnohem větší zátěží než pro mladého člověka. Druhá kapitola rozebírá změny, kterými člověk ve staří nevyhnutelně prochází a třetí kapitola se věnuje samotnému deliriu v intenzivní péči. Empirická část práce byla zaměřena na pacienty hospitalizované na jednotce intenzivní péče z důvodu operačního řešení zlomeniny. Pacienti byli pro možnost srovnání rozděleni do dvou skupin na pacienty, u kterých pooperační delirium nastalo a pacienty, kteří hospitalizaci zvládli bez deliria. Práce měla stanovené tři cíle, a to zjistit, který rizikový faktor se na rozvoji deliria podílí, zda pacienti, kteří během hospitalizace prodělali delirium jsou hospitalizovaní déle a zda může delirium ovlivnit způsob vedení anestezie. Výzkumným šetřením bylo zjištěno, že rizikový faktor v anamnéze má vliv na rozvoj deliria. Bylo potvrzeno, že pacienti s deliriem byli hospitalizováni déle než pacienti bez deliria a dále výzkum ukázal, že způsob vedení anestezie nemá vliv na rozvoj pooperačního deliria.

Abstract:

Postoperative delirium represents very serious complication at any patients' age level. The delirium is much bigger issue when regarding seniors affected by chronic illnesses. The delirium may cause acute brain damage. The primary signs are malfunction of cognition, memory, orientation, perception or hallucination, etc. These symptoms may lead to prolongation of hospitalization. Even though the scholarly literature and research are dealing with this issue, there is no reliable direction yet. First part of theory describes anesthesia options and the fact that geriatric patients tolerate the anasthesia much harder than the younger ones. The second part of theory analyzes changes which seniors must overcome and the third part deals with delirium intensive care itself. The empirical part of the thesis concentrates on patients who are hospitalized at intensive care unit because of different kinds of fracture. The patients were divided into two groups; the one who had suffered from deliria and those who had no postoperative complications. The theses has three aims; to find out which factor affects delirium the most, to determine if after delirium patients are hospitalized for longer time and if the delirium may affect the way of handling the anesthesia. The research shows that delirium patients were hospitalized for longer time and the way of handling the anasthesia does not affect postoperative delirium.
