Thoracic injuries in prehospital care

Abstract:

Poranění hrudníku patří mezi časté a život ohrožující úrazy. Mezi nejčastější příčiny patří dopravní nehody, pády z výšky a sporty (cyklistika, míčové hry, atd.). Tato práce je rozdělena na dvě základní části a to na část teoretickou a část praktickou. V teoretické části se nejprve věnuji anatomii, fyziologii, poranění hrudníku, které rozděluji na poranění kostí, svalů a vnitřních orgánů. Setkáváme se tu s poraněními typu zlomená žebra, pneumothorax, komoce, kontuze plic nebo srdce. Dále zranění neslučitelná se životem typu ruptura aorty, odtržení srdce od velkých cév, atd. Zranění se obecně projevují bolestivostí, dušností, krvácením, ale každý orgán má své specifické příznaky. K diagnostice v PNP slouží pouze naše ruce a zrak. Informace nám doplní mechanismus úrazu, anamnéza, neurologické vyšetření a monitorace ZŽF. Definitivní diagnózu nám určí až přístroje v nemocnici (RTG, CT, NMR, SONO, aj.). Léčbu zahajujeme zajištěním a monitorací základních životních funkcí. Následuje léčba farmaky a oxygenoterapie. Součástí terapie je také šetrný transport do zdravotnického zařízení. Druh léčby se odvíjí od stavu pacienta. Praktická část je věnována kazuistikám s řešením stavu pacienta od převzetí výzvy, ošetření a předání do zdravotnického zařízení.

Abstract:

Chest trauma belongs among frequent and life-threatening injuries. Motor vehicle crashes, falls and sports like cycling, ball game et al. are common causes of chest injuries. This dissertation is divided into two basic parts-theoretical and practical. Theoretical part is devoted to anatomy, physiology, chest injuries including bone fractures and soft tissue injuries. Specific types of chest trauma include rib fractures, pulmonary injury like pneumothorax, hemothorax, pulmonary and heart contusion , also injuries incompatible with life as aortic rupture, rip heart away from great vessels, etc. Injuries generally manifests with soreness, breathless, bleeding, however each organ has one´s own specific symptoms. Only our hands and sight serve to diagnostics in pre-hospital care. Another information for us are mechanism of accident , case, anamnesis, neurological examination and monitor vital signs. Up to hospital device determines final diagnosislike x-ray, CT scan, MR scan, Ultrasound etc. Prehospital therapy starts with provision and monitor vital signs, next drugs treatment and oxygen therapy.Protective transport is important part of therapy. kind of therapy unfold from patient´s condition. In practical part are individual cases with resolutions from taking emergency call, through medical treatment to delivery to hospital care.
