Probiotics and bacterial translocation in patients with liver cirrhosis

Abstract:

Střevní bakteriální přerůstání hraje důležitou roli u nemocných s jaterní cirhózou. Zvyšuje střevní propustnost a translokaci gramnegativních mikroorganismů skrze střevní stěnu do systémového oběhu. Tyto změny mohou vést ke vzniku infekce, zhoršení jaterních funkcí a rozvoji encefalopatie. Probiotika, nepatogenní živé mikroorganismy lidského původu, se mohou uplatnit v prevenci i léčbě těchto komplikací. Cílem naší práce bylo zjistit, zda by bylo možné a přínosné u pacientů s jaterní cirhózou ovlivnit střevní bakteriální osídlení podáváním probiotického kmene Escherichia coli Nissle, zda dojde ke snížení hladiny endotoxinu jako markeru bakteriální translokace a ke zlepšení jaterních funkcí. Ve skupině cirhotiků užívající probiotikum došlo ke statisticky významnému zlepšení mikrobiologického nálezu ve stolici: snížení počtu potenciálních patogenů, nárůstu laktobacilů a bifidobakterií. Při stanovení endotoxémie jsme nalezli trend naznačující možnou statistickou významnost při větším počtu nemocných. Jaterní funkce byla hodnocena Child-Pughovou klasifikací. Po podání E. coli Nissle dochází ke snížení počtu bodů v Child-Pughově klasifikací, hodnoty však nejsou statisticky významné. Probiotika mohou normalizací a zlepšením střevního osídlení zabránit bakteriálnímu přerůstání, omezit bakteriální translokaci, snížit hladinu endotoxinu, a tak zabránit životohrožujícím infekčním komplikacím, jimiž jsou nemocní s jaterní cirhózou ohroženi.

Abstract:

The bacterial overgrowth in the small intestine is very important in chronic liver diseases in consequence of contamination of gramnegative microorganisms. Bacterial overgrowth causes an increase of intestinal permeability, bacterial translocation and systemic endotoxemia. These changes can lead to deterioration of liver functions, development of infections and liver encefalopathy. Probiotics, live non-pathogenic microorganisms of human origin, might prevent these complications by restoring the intestinal colonization. The aim of our study was to assess the intestinal microflora by probiotic strain Escherichia coli Nissle and verify whether it may reduce the level of endotoxin as a marker of bacterial translocation, improve liver functions and minimal liver encefalopathy. We proved that Escherichia coli Nissle given to the patients with liver cirrhosis significantly restores intestinal microflora including lactobacilli and bifidobacteria, declines endotoxemia and potentially improves liver functions
