Metabolic changes in bariatric patients after the implementation of intragastric balloon

Abstract:

Tato bakalářská práce se zabývá obezitou a jejími komplikacemi, základními parametry hodnocení obezity a možnostmi její léčby se zaměřením na bariatrické chirurgické výkony a endoskopickou metodu implantace intragastrických balónů. V další části teoretické práce je uveden přehled jednotlivých hormonů, kterými se dále, spolu s parametry lipidového a sacharidového metabolismu, zabývá praktická část této práce. Jsou to leptin, ghrelin a adiponektin. Tyto hormony se účastní regulace příjmu potravy a regulace lipidového a sacharidového metabolismu. Cílem praktické části této práce je vyhodnocení účinků implantace intragastrického balónu Medsil? na parametry lipidového a sacharidového metabolismu (glukóza, glykovaný hemoglobin, triacylglyceroly, celkový cholesterol, HDL a LDL cholesterol, leptin, ghrelin, adiponektin) u skupiny pacientů interní kliniky Fakultní nemocnice Ostrava. Sledované parametry byly vyhodnocovány před implantací intragastrického balónu Medsil? a po třech a šesti měsících po jeho implantaci. Ke stanovení koncentrací vybraných parametrů bylo použito různých metod, a to především spektrofotometrických (glukóza, triacylglyceroly, celkový cholesterol, HDL a LDL cholesterol), ELISA (leptin, ghrelin, adiponektin) a metody vysokoúčinné chromatografie pro stanovení glykovaného hemoglobinu.

Abstract:

This bachelor thesis deals with obesity and its related complications, main parameters of obesity assessment and options of its treatment focusing on bariatric surgical procedures and the endoscopic method of intragastric balloon implantation. There is also an overview of hormones leptin, ghrelin and adiponectin given whereas the stated hormones are investigated together with the parameters of lipid and carbohydrate metabolism in the practical part of this thesis. These hormones are involved in food intake regulation and regulation of lipid and carbohydrate metabolism. The aim of the practical part of this bachelor thesis is the evaluation of the effect of the intragastric balloon Medsil? implantation on parameters of lipid and carbohydrate metabolism (glucose, glycated hemoglobin, total cholesterol, HDL and LDL cholesterol, leptin, ghrelin, adiponectin) in a group of patients from the Internal Clinic, University Hospital of Ostrava. All parameters were evaluated prior to implantation of intragastric balloon Medsil?, and after three and six months after its implantation. Concentrations of glucose, triacylglycerols, total cholesterol, HDL and LDL cholesterol were determined by spectrophotometry, concentrations of leptin, ghrelin and adiponectin were determined by ELISA method. Concentrations of glycated hemoglobin were determined by high performance liquid chromatography.
