Epidemiology of colorectal cancer in young adults in the Czech Republic

Abstract:

Rakovina tlustého střeva neboli kolorektální karcinom představuje v České republice třetí nejčastější nádorové onemocnění. Zavedením screeningového programu kolorektálního karcinomu došlo k poklesu incidence a mortality u osob nad 50 let. V posledních letech se u mladších osob žijících v zahraničí ukazuje opačný trend. Tato bakalářská práce by měla ověřit, zda se tento trend projevuje i u mladších osob v České republice. V teoretické části se včetně základního popisu nádorového onemocnění a jeho epidemiologie a prevence vyskytují také informace o screeningových programech kolorektálního karcinomu v zemích Evropské unie. Součástí teoretické části je rešerše zahrnující výsledky studií, které se již zabývaly podobnou problematikou jako předložená bakalářská práce. V teoretické části byly také popsány metody pro hodnocení epidemiologických trendů. Hlavním cílem bylo vybrat metodu pro hodnocení epidemiologických trendů. Pro hodnocení dostupných agregovaných dat z Národního onkologického registru byla aplikována metoda joinpoint regrese. Výsledky této práce ukazují na zvyšující se incidenci kolorektálního karcinomu u osob ve věku 20–29 let. U mladších osob 50 let v posledních letech převládá diagnostika kolorektálního karcinomu v pokročilém stádiu. Získané výsledky v této bakalářské práci byly diskutovány s ohledem na zavedený screeningový program v ČR s možností snížení věkové hranice pro screening. Také byly srovnávány s výsledky v jiných zemích.

Abstract:

Colorectal cancer is the third most common cancer in the Czech Republic. The introduction of a colorectal cancer screening program has reduced the incidence and mortality of people over 50 years of age. In recent years, the opposite trend has been shown in the age group of young people living abroad. This bachelor thesis should verify, whether this trend is reflected in younger people group, living in the Czech Republic. The theoretical part includes a basic description of cancer and its epidemiology and prevention, and also information on colorectal cancer screening programs in the countries of the European Union. The theoretical part comprises a search including the results of studies that have already dealt similar problems as the presented bachelor thesis. The theoretical part were also described methods for evaluating epidemiological trends. Main of the aims was to select a method for the evaluation of epidemiological trends. The joinpoint regression method was applied to the provided aggregated data from the National Cancer Registry. The results of this work show an increasing incidence of colorectal cancer in ČR aged 20-29 years. In people under the age of 50, the diagnosis of advanced colorectal cancer has prevailed in recent years. The obtained results in this bachelor thesis were discussed with regard to the established screening program in the Czech Republic, with the possibility of lowering the age limit for screening. They were also compared with the results from the other countries.
