Palliative Care in Intensive Medicine and Ethical Issues related to Patients Categorization

Abstract:

Diplomová práce se zabývá problematikou paliativní péče v intenzivní medicíně a etickým problémům spojených s kategorizací pacientů. V teoretické části jsou vymezeny základní pojmy jako: paliativní péče, práva pacientů v oblasti paliativní péče, kategorizace pacientů v intenzivní péči, potřeby pacientů a jejich rodinných příslušníků, bolest a její léčba, analgezie a sedace na jednotce intenzivní péče, fáze nevyléčitelného onemocnění, fáze vyrovnávání se s nevyléčitelnou nemocí a ztrátou, fáze umírání, zásady správné komunikace s pozůstalými a etické problémy intenzivní péče. V praktické části práce zkoumá metodou dotazníkového šetření názory sester a lékařů na problémy v této oblasti. Na základě názorů respondentů byla vytvořena doporučení pro praxi, která by mohla změnit úroveň paliativní péče v ČR.

Abstract:

The diploma thesis deals with the issue of palliative care in intensive medicine and ethical problems connected with patient categorization. The theoretical part defines basic terms such as: palliative care, patients' rights in palliative care, categorization of patients in intensive care, needs of patients and their family members, pain and its treatment, analgesia and sedation in the intensive care unit, phase of incurable disease, the stage of dealing with the incurable illness and loss, the dying phase, the principles of good communication with the bereaved and the ethical problems of intensive care. In the practical part of the thesis, the questionnaire survey examines the opinions of nurses and doctors on problems in this area. Based on the respondents' opinions, recommendations were made for practice that could change the level of palliative care in the Czech Republic.
