Needs of the patients in palliative care

Abstract:

Cíl: Cílem této bakalářské práce bylo zjistit důležitost bio-psycho-sociálních a spirituálních potřeb pacientů v paliativní péči. Dále pak zjistit, které potřeby hodnotí pacienti jako nenaplněné. Metodika: Sběr dat pro průzkumné šetření byl proveden metodou standardizovaného dotazníku pro hodnocení potřeb pacientů v paliativní péče - PNAP (Patients Needs Assessment in Palliative Care), který je původním českým dotazníkem. Zkoumaný soubor tvořili pacienti, u nichž pro závažnost zdravotního stavu a prognózu byla ukončena kurativní léčba a započata paliativní péče. Průzkumné šetření probíhalo v Hospici Frýdek-Místek po dobo tří měsíců. Výsledky: Průzkumným šetření byly identifikované problematické a neuspokojivě saturované potřeby. Nejvyšší důležitosti hodnotili pacienti potřeby v doméně respekt a podpora ZP a zároveň jako nejlépe saturovanou doménu. Vysokou důležitosti hodnotili pacienti také doménu autonomie. Nejmenší důležitost přikládali pacienti potřebám v doméně religiózní potřeby. Jako nejméně saturované, pacienti vnímali potřeby v doméně fyzické symptomy - únava, spánek, únik moči, únik stolice, dušnost, pocení a návaly horka, dále potřeby v doméně autonomie - schopnost soustředit se, pokračovat v obvyklých aktivitách, strach ze závislosti na pomoci druhých, potřeby v doméně smysl života a smíření, které byly nejhůře saturované ze všech domén a finanční zabezpečení v doméně sociální oblast. Průzkumné šetření prokázalo souvislost mezi pohlavím a potřebami pacientů, mezi onemocněním a potřebami pacientů a mezi věkovými skupinami a potřebami pacientů. Závěr: Zaměřením se na problematické oblasti lidského života, nalezením nejlepšího způsobu vedoucího ke zlepšení jejich stavu, můžeme zvýšit spokojenost pacientů a zároveň zlepšit kvalitu jejich života.

Abstract:

Aim: The aim of this Bachelor thesis was to learn the importance of bio-psycho-social and spiritual needs of patiens in palliative care as well as to discover which needs are assessed as unfulfilled by patients. Methods: The data collection for the survey research was carried out using the method of standardised questionnaire for assessing the needs of patiens in palliative care - PNAP (Patiens Needs Assessment in Palliative Care) which is an original Czech questionnaire. The file under review copmprised patiens whose curative care was finished because of the seriousness of the state of health and prognosis and palliative care was started. The research survey took place in the Frýdek-Místek Hospice within three months. Outcomes: Problematic and unsatisfactorily saturated needs were identified using a research survey. The patients gave the highest priority to the needs in the domain of respect and support of the health insurance company and at the same it was the best saturated domain. The patients evaluated the autonomy domain as highly important as well. They gave the lowest priority to the needs in the domain of religious needs. As the least saturated, they perceived the needs in the domain of physical symptoms (tiredness, sleep, incontinence, fecal incontinence, shortness of breath, sweating and hot flushes) and the needs in the domain of autonomy (the ability to concentrate, continue common activities, fear from dependence on the help of the others), needs in the domain of the meaning of life and conciliation which were the worst saturated from all domains and financial security in the social domain. The survey research proved a connection between gender and patiens´needs, between a disease and patients´needs and between age groups and patients´needs. Conclusion: By focusing on the problematic areas of human life and finding the best way leading to the improvement of their state, we can increase the contentment of patients and at the same time improve the quality of their lives.
