Perioperative care of patients undergoing cardiac surgery with a focus on DM

Abstract:

Diplomová práce se podrobně zabývá specifiky péče zaměřené na potřeby pacientů trpících diabetem mellitem během perioperačního období. Strukturována je do teoretické a výzkumné části. Teoretická část poskytuje přehled o diabetu mellitu, jeho vlivu na perioperační rizika a komplikace, a také popisuje specifika perioperační péče v kardiochirurgii s ohledem na tyto pacienty. Výzkumná část se zaměřuje na analýzu současných postupů v perioperační péči o diabetiky a identifikuje možné problematické oblasti. Výzkum odhalil, že pacienti s diabetem mellitem čelí v perioperačním období zvýšeným rizikům, což vyžaduje specifické postupy, jako je pečlivá glykemická kontrola, přizpůsobená inzulinová terapie, důkladné preventivní opatření proti infekcím a posílená edukace pacientů. Diskuze ukazuje, že zásadním prvkem je komplexní příprava pacienta před operací, která zahrnuje edukaci v oblasti péče o ránu, výživy a celkového managementu diabetu mellitu. Výsledky výzkumu rovněž poukazují na nedostatky v informovanosti pacientů a nutnost posílit multidisciplinární přístup včetně zapojení diabetologické sestry do edukačního procesu. Diplomová práce přichází s konkrétními doporučeními pro klinickou praxi, která mají za cíl zlepšit výsledky léčby a kvalitu života pacientů s diabetem. Zahrnují zlepšení v edukaci a přípravě pacientů, zvýšenou pozornost na glykemickou kontrolu a individuálně přizpůsobenou inzulinovou terapii, důraz na prevenci infekcí a podporu zdravého životního stylu po operaci. Navrhuje se další výzkum, který by se zaměřil na dlouhodobý dopad těchto zlepšení a hledal optimální frekvenci a metody edukace pro diabetické pacienty, aby bylo možné tyto zjištěné nedostatky efektivně řešit.

Abstract:

This thesis delves into the specifics of care focused on the needs of patients suffering from diabetes mellitus during the perioperative period. It is structured into theoretical and research parts. The theoretical section provides an overview of diabetes mellitus, its impact on perioperative risks and complications, and describes the specifics of perioperative care in cardiothoracic surgery considering these patients. The research part focuses on analyzing current practices in perioperative care for diabetics and identifies possible problematic areas. The research revealed that patients with diabetes mellitus face increased risks during the perioperative period, necessitating specific procedures such as careful glycemic control, tailored insulin therapy, thorough preventive measures against infections, and enhanced patient education. The discussion indicates that a crucial element is the comprehensive preparation of the patient before surgery, including education in wound care, nutrition, and overall management of diabetes mellitus. The research results also highlight deficiencies in patient information and the need to strengthen a multidisciplinary approach, including the involvement of the diabetes nurse in the educational process. The thesis comes with specific recommendations for clinical practice aimed at improving treatment outcomes and the quality of life of patients with diabetes. These include improvements in patient education and preparation, increased attention to glycemic control and individually tailored insulin therapy, emphasis on infection prevention, and support for a healthy lifestyle after surgery. Further research is suggested to focus on the long-term impact of these improvements and to find the optimal frequency and methods of education for diabetic patients to effectively address these identified gaps.
