Physiotherapy after LCA injury of the knee joint

Abstract:

Úvod: V bakalářské práci jsem se věnoval problematice fyzioterapie po poranění kolenního kloubu. Mým pacientem byl aktivní sportovec, konkrétně hráč fotbalu. Primárním důvodem výběru pro mě bylo snažit se poznat problematiku poranění kolenního kloubu u aktivního sportovce a sledovat několikaměsíční postup návratu zpět do plné zátěže. Možností pro mě byla také následná spolupráce s kondičním trenérem fotbalového klubu. Mou motivací byla možnost aktivního zapojení a možnosti zlepšení rehabilitačních postupů a prevenčních skupinových cvičení ke snížení rizika vzniku podobných zranění. Cíl práce: Cílem této bakalářské práce je přispět k optimálnímu řešení problematiky volby fyzioterapeutických postupů po poranění v oblasti kolenního kloubu a jeho postupnému návratu do plné sportovní zátěže. Metodika: Tématem úvodní části práce je shrnutí rešerší a teoretických poznatků. Kazuistika mladého, 17letého fotbalisty, po poranění LCA je součástí další části práce. Součástí kazuistiky je vstupní kineziologický rozbor, díky kterému jsem následně vytvořil krátkodobý plán fyzioterapie. Ten byl složený celkově z 12 přibližně 60 - 90minutových terapií. Kontrolní měření svalové síly, rozsahu pohybu a obvodů DKK proběhlo ve 4. týdnu naší intervence s probandem. Závěrečným bodem kazuistiky bylo zpracování výstupního kineziologického rozboru a navržení dlouhodobého plánu fyzioterapie, které probíhalo ve spolupráci s již zmíněným kondičním trenérem. Výsledky: S probandem se nám během společné spolupráce podařilo zredukovat otok kloubu a postupně uvolnit měkké tkáně především v okolí kolenního kloubu. Za velmi důležité pokládám také zvýšení svalové síly a dosáhnutí plné extenze a flexe v kolenním kloubu. Úspěšným výsledkem bylo také zlepšení centrace kořenových kloubů, reedukace dechového stereotypu a obnova stereotypu chůze. V terapii jsem proto využil konceptu DNS, PNF, Brügger koncept a další. Dále také gumovou pásku Flossband, Blackroll, fyzikální terapii atd. Po dokončení byla z pacienta cítit zjevná motivace a chuť pracovat dále na návratu do plného zatížení. Závěry: Musím vyzdvihnout pracovitost a pečlivost pacienta při našich společných setkáních. Díky tomuto přístupu se zvýšila úspěšnost naší společné terapie. Pevně věřím, že touto prací jsem alespoň částečně mohl přispět k volbě vhodných fyzioterapeutických metod.

Abstract:

Introduction: In my bachelor's thesis, I followed up on the issue of physiotherapy after a knee joint injury. My patient was an athlete, a football player specifically. The main reason I chose this topic was to try understand the issue of knee joint injuries. Also follow the several-month process of returning to full load. An option for me was also cooperate with the football club's physical coach. My motivation was the possibility of active involvement and the possibility of improving rehabilitation methods. The objective: The goal of this bachelor's thesis is to contribute to the optimal solution of the issue of choosing physiotherapy methods after an injury in the area of the knee joint and return to full sports load. Methodology: The introductory topic of the bachelor's thesis is a summary of research and theoretical findings. The case report of a young 17 years old football player after an LCA injury is another part of the work. Part of the case report is an initial kinesiological analysis, then I created a short-term physiotherapy plan. The short-term plan was consisted of a total of 12 therapies - approximately 60-90 minutes. In the 4th week of our cooperation with the patient were realised control measurements of muscle strength, range of motion and legs circuits. The final point of the case study was the processing of the final kinesiology analysis and the design of a long-term plan of physiotherapy, which took place in cooperation with the already mentioned fitness trainer. Results: During our collaboration with the patient, we managed to reduce the swelling of the joint and gradually released the soft tissues, especially around the knee joint. I also consider increasing muscle strength and achieving full extension and flexion in the knee joint to be very important. The successful result was also the improvement of the centering of the root joints, the re-education of the breathing stereotype and the restoration of the walking stereotype. That's why I used the DNS concept, PNF, Brügger concept and others in therapy. I used Flossband, Blackroll, physical therapy, etc. After finishing, he was clearly motivated to work on getting back to full load. Conclusions: I have to appreciate the patient's hard work and diligence during our meetings together. Thanks to this approach, the success of our joint therapy has increased I believe that with this bachelor's thesis I could partially contribute to the choice of suitable physiotherapy methods.
