Respiratory physiotherapy in Parkinson's disease

Abstract:

Úvod: Parkinsonova choroba je progresivní neurodegenerativní onemocnění projevující se motorickými a non-motorickými příznaky. Incidence Parkinsonovy choroby v ČR roste. Hlavní příčinou úmrtí pacientů s tímto onemocněním je pneumonie z aspirace. Z tohoto hlediska je důležité udržet hygienu dýchacích cest vhodnou volbou fyzioterapeutické intervence. Cíl práce: Cílem bakalářské práce je přispět k řešení problematiky respirační fyzioterapie u Parkinsonovy choroby. Metodika: Teoretická část obsahuje odborné poznatky k danému tématu. Praktická část byla zpracována formou kazuistiky pacienta v pokročilém stádium Parkinsonovy choroby. Na základě vstupního vyšetření a cílů pacienta byl stanoven krátkodobý fyzioterapeutický plán, který byl realizován v rámci osmi šedesátiminutových terapií s frekvencí jedna terapie týdně. V rámci terapie jsem využila tyto metody a postupy: respirační fyzioterapie, měkké techniky, mobilizační techniky, postizometrická relaxace, postfacilitační strečink, Stabilizační a mobilizační systém dle Smíška, cvičení pro osoby s Parkinsonovou nemocí. Každé tři týdny byla přeměřena síla dýchacích svalů a vrcholový průtok kašle a dle výsledku přenastavena terapie s respiračním trenažerem Expiratory muscle strenght trainer 150. Po osmi týdnech byl proveden výstupní kineziologický rozbor a stanoven dlouhodobý fyzioterapeutický plán. Výsledky: Oproti vstupnímu vyšetření nedošlo k mnoha změnám, dalo se to však přepokládat vzhledem k diagnóze a stadiu onemocnění. Nejvýznamnější výsledkem bylo zlepšení maximální výdechového tlaku o 60 cmH2O a vrcholového průtoku kašle o 109 l/min. Závěr: Z mého subjektivního pohledu bylo zásadní zjištění, že lze pomocí tréninku s respiračním trenažerem Expiratory muscle strenght trainer 150 zlepšit sílu exspiračních svalů a zlepšit tak efektivitu kašle. Zpracování této bakalářské práce jsme si prohloubila teoretické znalosti o Parkinsonově nemoci a respirační fyzioterapii a zároveň jsem si osvojila některé vyšetřovací metody a fyzioterapeutické postupy.

Abstract:

Introduction: Parkinson's disease is a progressive neurodegenerative disease with motor and non-motor symptoms. The incidence of Parkinson's disease in the Czech Republic is increasing. The main cause of death in patients with this disease is aspiration pneumonia. From this point of view, it is important to maintain airway hygiene by choosing a suitable physiotherapeutic intervention. The aim: The aim of the bachelor thesis is to contribute to solving the problem of respiratory physiotherapy in Parkinson's disease. Methods: The theoretical part contains expert knowledge on the given topic. The practical part was prepared in the form of a case study of a patient in an advanced stage of Parkinson's disease. On the basis of the initial examination and the patient's goals, a short-term physiotherapy plan was established, which was implemented within the framework of eight sixty-minute therapies with a frequency of one therapy per week. As part of the therapy, I used the following methods and procedures: respiratory physiotherapy, soft techniques, mobilization techniques, post-isometric relaxation, post- facilitation stretching, Stabilization and mobilization system according to Smíšek, exercises for people with Parkinson's disease. Every three weeks, respiratory muscle strength and peak cough flow were measured, and therapy with the Expiratory muscle strenght trainer 150 was adjusted according to the results. After eight weeks, an output kinesiological analysis was performed and a long-term physiotherapy plan was established. Results: Compared to the initial examination, there were not many changes, but this could be assumed due to the diagnosis and stage of the disease. The most significant result was an improvement in maximal expiratory pressure by 60 cmH2O and peak cough flow by 109 l/min. Conclusion: From my subjective point of view, it was essential to discover that the strength of the exspiratory muscles can be improved by training with the Expiratory muscle strenght trainer 150, thus improving the effectiveness of coughing. By working on this bachelor thesis, I deepened my theoretical knowledge of Parkinson's disease and respiratory physiotherapy, and at the same time I have learned some examination methods and physiotherapy procedures.
