The importance of the rehabilitation procedures in patients with hemiplegic shoulder pain

Abstract:

Diplomová práce se zabývá rehabilitací horní končetiny u neurologických pacientů po cévní mozkové příhodě se syndromem hemiparetického ramene. Cílem bylo zhodnocení vlivu klasické rehabilitace a rehabilitace doplněné o speciální techniky na neurofyziologickém podkladě zaměřené na pletenec ramenní, měkké a mobilizační techniky a kinesiotaping na bolestivost, funkci a rozsah pohybu v ramenním kloubu postižené horní končetiny. K objektivizaci výsledků byla zvolena Testovací škála pro bolestivé rameno Chedoke-McMaster (Chedoke-McMaster Stroke Assessment, CMSA), Dotazník bolesti a disability ramene SPADI (Shoulder Pain and Disability Index, SPADI), Vizuální analogová škála bolesti (Visual Analogue Scale, VAS) a klasická goniometrie. Soubor pacientů se syndromem bolestivého ramene se skládal z 23 jedinců, kteří byli náhodně rozděleni do kontrolní a výzkumné skupiny. Testování proběhlo před zahájením rehabilitace a po jejím ukončení. Ve výzkumné skupině byl potvrzen statisticky významný rozdíl u klasické goniometrie ve všech směrech pohybu, u CMSA testu, a u VAS měřené při zátěži. V rámci SPADI testu bylo zaznamenáno statisticky signifikantní zlepšení bolesti a disability u celého souboru pacientů. U kontrolní skupiny došlo k statisticky významnému zlepšení pouze u aktivního rozsahu pohybu ramenního kloubu do flexe měřené klasickou goniometrií. Mezi skupinami nebyl prokázán žádný statisticky významný rozdíl. Všechny výsledky byly hodnoceny na hladině statistické významnosti α=0,05.

Abstract:

The diploma thesis deals with the rehabilitation of the upper extremities in neurological stroke patients with hemiplegic shoulder pain. The aim was to evaluate the effect of classical rehabilitation and rehabilitation supplemented with special neurophysiological techniques focused on the shoulder, further soft and mobilization techniques and shoulder kinesiotaping on pain, function and range of motion in the disabled shoulder joint. Chedoke-McMaster Stroke Assessment (CMSA), Shoulder Pain and Disability Index (SPADI), Visual Analogue Scale (VAS) and goniometry were chosen to target results. The group of patients with hemiplegic shoulder pain consisted of 23 individuals, who were randomly divided into a control group and a research group.The research group was confirmed to have a statistically significant difference in goniometry in all directions of active range of motion, in the CMSA test and in VAS measured in the exercice. Within the SPADI test, there was a statistically significant improvement in pain and disability in the entire group of patients with hemiplegic shoulder pain. In the control group there was a statistically significant improvement only in the active range of motion of shoulder joint movement to flexion measured by goniometry. There was no statistically significant difference between the groups. All evaluations were performed at a statistical significance α=0,05.
