Occupational therapy in extensor tendon hand injury

Abstract:

Název: Ergoterapie u šlachového poranění extenzorů ruky Cíl: Cílem bakalářské práce je poukázat na možnosti a důležitost ergoterapeutické intervence u šlachového poranění extenzorů ruky Metodika: Tato práce je zpracována formou kazuistiky klienta po šlachovém poranění extenzorů ruky. Klient byl hospitalizován v Ústavu chirurgie ruky a plastické chirurgie ve Vysokém nad Jizerou a zde proběhla čtrnáctidenní časná rehabilitace tři týdny po vzniku poranění. V úvodní části práce jsem provedla analýzu odborné literatury a posbírala teoretické poznatky, které se týkají anatomie a kineziologie předloktí a ruky, rozdělní šlachového poranění extenzorů a rehabilitace v jednotlivých zónách po šlachových poraněních extenzorů. Věnovala jsem se zejména ergoterapii po takovýchto poraněních, i když literatura je v této oblasti velmi nedostačující. Hlavní část práce popisuje konkrétní postupy, které jsem v ergoterapeutické intervenci u mého klienta využívala. Hlavní část obsahuje anamnézu odebranou formou rozhovoru s klientem, vstupní a výstupní vyšetření, při kterém bylo provedeno měření délek a obvodů horní končetiny a rozsah pohybů v jednotlivých segmentech, svalový test, testování končetiny vybranými testy jemné motoriky a v neposlední řadě pozorování poraněné ruky. Na konci hlavní části mé bakalářské práce jsou uvedeny výsledky a porovnání výsledků před intervencí a po intervenci. Výsledky: Výstupním vyšetřením bylo zjištěno zejména snížení otoku a výsledkem je i velmi nevýrazná jizva růžového zbarvení bez značných výstupů. Dále bylo zjištěno zlepšení úchopů a zvýšení rozsahů pohybů zejména v rozsahu pohybů prstů. Rozdíly jsou vidět i ve svalovém funkčním testu, kde ale nelze posoudit výsledky objektivně, vzhledem k tomu, že při vstupním vyšetření nešlo testovat všechny segmenty. Dalším cílem bylo stanovit vhodné testy jemné motoriky po šlachových poraněních v časné fázi rehabilitace, zde se jako nejvhodnější jevily tři testy ze čtyř, a to Box and Block, The Minnesota Dexterity Test a Jebsen Taylor Hand Function test. Závěr: Z výsledků výstupního vyšetření je jasně viditelné, že ergoterapeutická intervence u klienta po šlachovém poranění extenzorů ruky byla velmi přínosná. S rukou je potřeba pokračovat v terapii i nadále, vzhledem k tomu, že funkční stav končetiny ještě není ideální, i s těmito výsledky může klient žít svůj obvyklý život, s drobným omezením. Nicméně pro dosažení lepších výsledků je vhodné i po skončení intervence pokračovat v autoterapii a ruku zapojovat do činností ADL.

Title: Occupational therapy in extensor tendon hand injury Obejctive: The aim of the bachelor thesis is to point out the possibilities and importance of occupational therapy intervention in extensor tendon hand injury Methodology: This thesis is processed in the form of a case report of the client after the tendon injury of the hand extenders. The client was hospitalized at the Institute of Hand Surgery and Plastic Surgery in Vysoké nad Jizerou and there he was fourteen days for early rehabilitation three weeks after injury. In the first part of my thesis I analyzed the literature and collected the theoretical knowledge concerning anatomy and kinesiology of forearm and hand, distribution of tendon wounds of extenors and rehabilitation in individual zones in extensor tendon injury. I have been particularly interested in occupational therapy following such injuries, although literature is very weak in this area. The main part of the thesis describes the specific procedures I have used in the occupational therapy intervention of my client. The main part consists of an anamnesis taken in the form of interview with the client, an entrance and an exit examination in which the measurements of the lengths and the limbs of the upper limb and the range of movements in individual segments, the muscle test, the limb testing by selected fine motor tests, and last but not least the observation of the injured hand. At the end of the main part of my bachelor thesis are presented results and comparison of the results before the intervention and after the intervention. Results: Output examination showed especially a reduction in swelling, resulting in a very faint scar of pink color without significant outcomes. In addition, improved gripping and increased movement ranges were found, especially in the range of finger movements. Differences can also be seen in the muscular function test, but the results can not be assessed objectively, given that all segments could not be tested at the initial examination. Another goal was to determine appropriate trials of fine motorics after tendon injuries in the early stage of rehabilitation, here three of four tests were the most appropriate: Box and Block, The Minnesota Dexterity Test and Jebsen Taylor Hand Function Test. Conclusion: It is clear from the results of the exit examination that the occupational therapy intervention with the client after extensor tendon hand injury was very beneficial. With the hand, it is necessary to continue the therapy as the functional condition of the limb is not yet ideal, even with these results, the client can live his usual life, with minor limitations. However, in order to achieve better results, it is advisable to continue selfmedication and engage in ADL activities after the end of the intervention.
