Vojta's method of reflex locomotion in painful shoulder syndrome

Abstract:

ABSTRAKT Úvod: Tématem mé bakalářské práce je použití terapeutického přístupu Vojtovy metody reflexní lokomoce u impingement syndromu ramenního kloubu u pacientů s touto diagnózou. Cíl práce: Cílem mé práce je přispění k problematice impingement syndromu ramenního kloubu pomocí terapie Vojtovou metodou reflexní lokomoce. Metodika: Hlavní část mé bakalářské práce se zabývá kazuistikou pacientky po ASK (artroskopii) levého ramenního kloubu z důvodu subakromiální dekomprese. S pacientkou jsem spolupracovala 1 měsíc, vždy 2x týdně, tedy absolvovala 8 návštěv. Před zahájením mé fyzioterapeutické intervence jsem provedla vstupní kineziologický rozbor (vyšetřila jsem aspekcí, palpací, goniometrii HKK , antropometrii HKK, svalový test dle Jandy (1996), zkrácené struktury, speciální vyšetřovací testy: Painfull arc, test, testování bolesti pomocí objektivní analogové škály bolesti (VAS). Dále testy HSSp, test nitrobřišního tlaku, brániční test - dle Koláře (2009), vyšetření stoje a chůze) na jehož základě jsem vypracovala krátkodobý fyzioterapeutický plán. V průběhu fyzioterapeutických intervencí jsem provedla jedno kontrolní vyšetření po čtvrté terapii. Hlavní náplní terapie bylo využití Vojtovy metody reflexní lokomoce a následné cvičení v UKŘ (uzavřený kinematický řetězec) a OKŘ (otevřený kinematický řetězec). Následně bylo provedeno cvičení posturálních funkcí ve vývojových řadách Výsledky: U pacientky došlo ke zmírnění bolestí levého ramenního kloubu ve všech pohybech, zlepšení rozsahu aktivního pohybu do flexe o 20°, extenze o 10°, abdukce o 30°. Zlepšila se svalová síla ramenního kloubu ve všech pohybech o jeden stupeň. Zlepšila se pohyblivost páteře Ottovy reklinační i inklinační distance o 0,5 cm. Zlepšila se trupová stabilizace aktivním udržením sagitální stabilizace. Závěry: Tato práce mne obohatila o nové teoretické poznatky diagnózy impingement syndrom a pomohla mi lépe pochopit souvislost mezi funkčním a strukturálním postižením pletence ramenního.

Abstract:

SUMMARY Introduction: The topic of bachelor thesis is the use a therapeutic approach to Vojta Method Reflex Locomotion in the Impingement Syndrome Shoulder joint in patients with this diagnosis. The objective: The aim of work is to contribute to the problem of impingement the shoulder joint syndrome using Vojta Therapy by the Method of Reflex Locomotion. Methodology: The main part of bachelor thesis deals with a case patient after ASK (arthroscopy) of the left shoulder joint due to subacromial decompression. I worked with the patient for 1 month, twice a week, so she completed 8 visits. Before starting my physiotherapeutic intervention, I performed an initial kinesiological analysis (I examined aspection, palpation, goniometry, anthropometry, Janda muscle test (1996), shortened structures and special examination tests: Painful arch, test, testing of pain scales using objective analog pain (VAS). Further tests . intra-abdominal pressure test, diaphragmatic test - according to Kolář (2009), examination of standing and walking on the basis of which. I developed a short term physiotherapy plan. During the physiotherapeutic interventions, I perforemd one follow up examination after the fourth therapy. The main content of the therapy was to use Vojta's Method of Reflex Locomotion and subsequent exercise in closed kinematic chain and opened kinematic chain, exercise of postural functions in developmental series, to improve range of motion and muscle strength and especially implementation of shoulder girdle into postural locomotor system. Before the end of the physiotherapeutic intervention, I performed a final kinesiological examination and subsequently created a long-term rehabilitation plan. Results: The patient had relief from pain in left shoulder joint in all movements. Improvement of the range of active movement to flexion by 20°, extension by 10°, abduction by 30°. Improved muscle strenght of the shoulder joint in all movements by one degrese. The backbone mobility of Otto reclining and inclination distance by 0,5 cm has improved. The overall stabilization by actively maintaining sagittal stabilization. Conclusions: This work enriched me with new theoretical findings in the diagnosis of impingement syndrome and helped me better understand the relationship between functional and structural shoulder girdle.
