Postižení pravé a levé srdeční komory u pacientů s akutním infarktem myokardu

Right and left ventricular impairment in patients with acute myocardial infarction

Abstract:

Práce vychází ze souboru 299 prospektivně zařazených pacientů s akutním infarktem myokardu s elevacemi ST úseku (STEMI), kteří byli léčeni primární perkutánní koronární intervencí (PPCI). Protokol práce byl veden dle Helsinské deklarace, byl schválen lokální etickou komisí a všichni pacienti podepsali informovaný souhlas. Cíle práce jsou následující: 1) Zjišťovali jsme, zda u pacientů, u kterých před provedením PPCI došlo k úplné normalizaci EKG (akutní infarkt myokardu s tranzientními elevacemi ST úseku, TSTEMI), došlo také k obnovení toku v infarktové koronární tepně, a srovnali jsme jejich dlouhodobou prognózu se STEMI pacienty, u kterých ischemické EKG změny přetrvávaly. 2) Hodnotili jsme výskyt a prognostických význam epizod hypotenze v subakutní fázi STEMI. 3) Dalším cílem práce bylo zjistit, jak souvisí EKG známky postižení pravé komory s lokalizací infarktu myokardu a postiženou koronární tepnou. Zjišťovali jsme také, jak souvisí celková doba ischemie STEMI pacientů s dlouhodobou prognózou a analyzovali jsme jednotlivé komponenty celkové doby ischemie. 4) V práci je podán i state-of-the-art přehled o možnostech diagnostiky a léčby infarktu myokardu pravé komory. V práci bylo prokázáno, že i přes normalizaci EKG nedošlo u 55% TSTEMI pacientů před provedením PPCI k obnovení průtoku infarktovou koronární tepnou, přičemž 5letá mortalita STEMI a TSTEMI pacientů se nelišila. Byl prokázán častý (31,4% pacientů) výskyt hypotenze v subakutní fázi STEMI, a to zejména u žen a pacientů s vysokou plazmatickou hladinou troponinu T. Signifikantně vyšší mortalitu měli pacienti, u kterých rozvoj hypotenze zabránil nasazení inhibitorů angiotenzin konvertujícího enzymu nebo blokátoru receptoru angiotenzinu II (p= 0,016). Zjistili jsme také, že EKG známky postižení pravé komory byly během akutní fáze STEMI přítomny u 66 pacientů (22,1%), přičemž se překvapivě téměř v polovině případů (31 pacientů) jednalo o STEMI přední stěny. U 47 pacientů (15,7%) trvala celková doba ischemie méně než 120 minut. Tito pacienti měli oproti ostatním signifikantně nižší mortalitu (5leté přežití 97,7% vs. 86.5%, p= 0,038) Na celkové době ischemie se nejvýznamnější měrou podílelo zpoždění způsobné pacientem (medián v souboru 90 minut). Z přehledu věnujícímu se současným možnostem diagnostiky a léčby postižení pravé komory u STEMI pacientů vyplývá, že i přes pokroky v diagnostice a léčbě zůstává postižení pravé komory významným prediktorem nepříznivé prognózy STEMI pacientů léčených PPCI.

Abstract:

We evaluated the data in a cohort of 299 consecutive, prospective enrolled patients with ST segment elevation myocardial infarction (STEMI) treated with primary percutaneous coronary intervention (PPCI). The protocol of the study was approved by the local Ethics Committee and all patients gave a signed informed consent according to the Helsinki Declaration. The aims of the work are following: 1) We evaluated the hypothesis, whether in patients with full normalization of ECG before PPCI (transient ST-elevation myocardial infarction - TSTEMI), the patency of the infarct related artery (IRA) has been achieved and their clinical outcome compared to patients with continuous ST-elevations. 2) We evaluated the occurrence and prognostic significance of the hypotension episodes during sub-acute phase of STEMI. 3) Our next aim was to assess the occurrence of ECG signs of right ventricular myocardial injury (RVMI) in relationship to ECG localization of STEMI and to IRA. Further we evaluated the relationship between the total ischaemic time and long-term mortality as well as the components of the total ischaemic time. 4) Our last aim was to present the review of the state-of-the-art options of diagnosis and treatment of the right ventricular myocardial infarction. We found, that despite normalization of ECG, the incidence of occluded IRA among TSTEMI patients is relatively high (55%) but the five year mortality of TSTEMI and STEMI patients did not differ. Our next observation was that hypotension episodes were common (31.4% of patients) in the sub-acute phase of PPCI treated STEMI patients, in particular in women and patients with higher values of troponin T. Patients, in whom hypotension prevented administration of angiotensin-converting enzyme inhibitors / angiotensin receptor blockers type II had significantly higher mortality (p=0.016). Further, the ECG signs of RVMI during acute phase of STEMI were found in 66 patients (22.1%), whereas, suprisingly, 31 patients suffered anterior-wall STEMI. There were 47 patients (15.7%) with total ischaemic time below 120 min. This cohort of patients had significantly better 5-year survival (97.7%) compared to the survival of other patients (86.5%, p= 0.038). Patient delay with a median of 90 min contributed substantially to the overall ischaemic time. Despite appreciable advances in the diagnosis and management of patients with RWMI, RV involvement in acute MI has remained an independent negative predictor of prognosis.
