Repetitive transcranial magnetic stimulation in the treatment of major depressive disorder

Abstract:

Úvod: Depresivní porucha představuje jednu z nejčastějších a ve svých důsledcích také nejzávažnějších duševních poruch. I přes vývoj nových antidepresiv stále zůstává skupina pacientů, pro které není farmakoterapie dostatečně účinná nebo z nějakých důvodů nemohou či nechtějí léky užívat. Existuje proto prostor pro rozvoj nových neurostimulačních terapeutických metod, mezi které patří i repetitivní transkraniální magnetická stimulace (rTMS). Mechanismus jejího účinku není přesně známý, ale bylo zjištěno, že vysokofrekvenční stimulace zvyšuje kortikální excitabilitu a metabolismus neuronů, kdežto nízkofrekvenční je snižuje. Je však pravděpodobné, že rTMS působí zprostředkovaně i na další struktury mozku, zejména limbické a paralimbické. Dle posledních studií a metaanalýz je prokázáno, že je její účinnost v akutní léčbě depresivní poruchy poměrně vysoká a srovnatelná s běžně užívanými antidepresivy. Přímé srovnání účinnosti rTMS a antidepresiv je však poměrně vzácné. Cíl: Hlavním cílem naší randomizované, jednoduše zaslepené studie bylo ověřit účinnost vysokofrekvenční rTMS nad oblastí levého dorzolaterálního prefrontálního kortexu (DLPFC) ve srovnání s běžně užívanými antidepresivy III. nebo IV. generace, vedlejším cílem studie bylo zjistit její schopnost ovlivnit kognitivní funkce depresivních nemocných opět ve srovnání se zmíněnými antidepresivy. Metodika: Do studie bylo zařazeno celkem čtyřicet pacientů (mužů) s diagnózou depresivní epizody nebo rekurentní depresivní poruchy, kteří byli náhodně rozděleni do dvou stejně velkých podskupin. Jedna podskupina byla po týdenní wash-out periodě léčena pomocí rTMS, druhá antidepresivy. Každý pacient se před léčbou a po ní podrobil zhodnocení psychopatologie pomocí škál HAMD a MADRS a neuropsychologickému vyšetření. Stimulace byla aplikována nad levý DLPFC o frekvenci 15 Hz, intenzitě 110% klidového motorického prahu, série pulsů trvala deset sekund, interval mezi nimi třicet sekund, celkový počet pulsů během sezení byl 1500 a počet sezení patnáct v průběhu tří týdnů. V kontrolní skupině byla podávána antidepresiva (SSRI, mirtazapin nebo kombinace venlafaxinu s mirtazapinem) v běžných dávkách. Výsledky: V podskupině léčené pomocí rTMS došlo k poklesu závažnosti psychopatologie ve škále HAMD z 21,15 (SD=3,63) na 9,05 (SD=3,76), ve škále MADRS z 28,95 (SD=4,25) na 13,00 (SD=4,89), což bylo v obou škálách statisticky významné (p=0,001). V podskupině léčené antidepresivy došlo k poklesu závažnosti psychopatologie ve škále HAMD z 21,05 (SD=2,79) na 9,55 (SD=3,24), ve škále MADRS z 29,70 (SD=3,92) na 13,55 (SD=3,93), což bylo rovněž v obou škálách statisticky významné (p=0,001). Rozdíly mezi podskupinami naopak statisticky významné nebyly. V rTMS podskupině bylo čtrnáct respondérů, z toho pět nemocných dosáhlo remise, v podskupině léčené antidepresivy bylo třináct respondérů, z toho šest nemocných dosáhlo remise. Co se týče kognitivních funkcí, v podskupině léčené pomocí rTMS došlo ke statisticky signifikantnímu zlepšení psychomotorického tempa (TMT A), odolnosti vůči percepční a psychické zátěži (CWT), slovní plynulosti (VFT), mnestických schopností (WMS-III) a exekutivních funkcí v kategorii perseverativní odezvy a perseverativní chyby (WCST). Po Bonferroniho korekci zůstalo signifikantní zlepšení mnestických funkcí a exekutivních funkcí v kategorii perseverativní odezvy. V podskupině léčené antidepresivy došlo ke statisticky signifikantnímu zlepšení odolnosti vůči percepční a psychické zátěži (CWT), slovní plynulosti (VFT) a mnestických schopností (WMS-III). Závěr: Podařilo se prokázat, že vysokofrekvenční rTMS nad oblastí levého DLPFC je stejně účinná jako běžně užívaná antidepresiva III. nebo IV. generace a v ovlivnění kognitivních funkcí se jeví jako účinnější. Tento závěr však musí být ověřen na větším souboru pacientů a možná s optimálněji nastavenými parametry stimulace.

Abstract:

Introduction: Depressive disorder is one of the most often and also most serious mental disorders. Although we have new antidepressants, there is a remaining group of patients, for whom the pharmacotherapy is not efficient enough or who cannot or don´t want to use antidepressants. So the development of new neurostimulation therapeutic methods is needed, one of these methods is also a repetitive transcranial magnetic stimulation (rTMS). The mechanism of its action is not fully clear, but it is known, that high-frequency stimulation increases cortical excitability and metabolism of neurons, on the contrary low-frequency stimulation decreases them. This method probably influences not only cortex, but indirectly also other brain structures, especially limbic and paralimbic ones. According to recent studies and meta-analyses the efficacy of rTMS in acute treatment of depression is quite high and comparable to the efficacy of common antidepressants. But direct comparisons between rTMS and antidepressants are quite rare. Aim: The main aim of our randomized, single-blinded study was to assess the efficacy of high-frequency rTMS over the left dorsolateral prefrontal cortex (DLPFC) in comparison with common antidepressants of the third or the fourth generation. The second aim of our study was to assess the effect of rTMS on cognitive functions and to compare it with the effect of antidepressants. Methods: Forty male patients diagnosed with depressive episode or recurrent depressive disorder were included into the study. They were randomised into two subgroups. One subgroup was treated after one-week wash-out period with rTMS, the other with antidepressants. Every patient was rated before and after the treatment using HAMD and MADRS scales and underwent neuropsychological assessment. The stimulation was administered over the left DLPFC, the stimulation frequency was 15 Hz, the intensity was 110% of resting motor threshold, one train lasted ten seconds, one intertrain lasted thirty seconds, the whole number of pulses during one session was 1500 and the number of sessions was fifteen in three weeks. Patients in control subgroup were treated with antidepressants (SSRIs, mirtazapine or combination of venlafaxine and mirtazapine) in standard daily doses. Results: The mean HAMD score decreased in the rTMS subgroup from 21.15 (SD=3.63) to 9.05 (SD=3.76), the mean MADRS score from 28.95 (SD=4.25) to 13.00 (SD=4.89), which was statistically significant (p=0.001) in both scales. The mean HAMD score in the control subgroup of patients treated with antidepressants decreased from 21.05 (SD=2.79) to 9.55 (SD=3.24), the mean MADRS score from 29.70 (SD=3.92) to 13.55 (SD=3.93), which was statistically significant (p=0.001) in both scales too. Difference between both subgroups was not statistically significant. In the rTMS subgroup there were fourteen responders, including five remitters, in the control subgroup there were thirteen responders, including six remitters. As far as cognitive functions are concerned, after rTMS we have found statistically significant improvement in psychomotor speed (TMT A), tolerance against perceptional and mental stress (CWT), verbal fluency task (VFT), memory (WMS-III) and executive functions in category of perseverative responses and perseverative errors (WCST). Improvement of memory and executive functions in category of perseverative responses remained significant even after Bonferroni correction. After treatment with antidepressants we have found statistically significant improvement in tolerance against perceptional and mental stress (CWT), verbal fluency task (VFT) and memory (WMS-III).
