Effect of caesarean section on breastfeeding in the early puerperium

Abstract:

Jak už název napovídá, tato práce řeší problematiku kojení po císařském řezu. V její první části jsou popsány zásady správné techniky kojení, faktory ovlivňující kojení, polohy při kojení a nejčastější problémy s kojením. Mimo kojení jsou v teoretické části popsány rozdíly mezi vaginálním porodem a císařským řezem, druhy anestezie při císařském řezu a vysvětlení raného šestinedělí. Hlavním cílem bylo zjistit, jaké nejčastější potíže s kojením mají ženy po porodu císařským řezem a jak se tyto problémy liší od problémů s kojením po vaginálním porodu. Data byla získávána pomocí anonymního dotazníku, který byl rozdán ženám, které rodily v termínu a byly třetí den po porodu. Žen bylo dohromady 115 z toho 81 bylo po vaginálním porodu a 34 po císařském řezu. V praktické části se zjistilo několik problémů s kojením, na které měl vliv způsob porodu. Jedním z problémů bylo úspěšné první přiložení. Prokázalo se, že ženy po císařském řezu jsou méně úspěšné při prvním přiložení (53,0 %), než ženy po vaginálním porodu (36,3 %). Nejvýznamnější rozdíl byl ale v podávání příkrmů, kdy 47,1 % dotazovaných žen po císařském řezu podávalo příkrmy a jen 17,5 % žen podávalo příkrmy po vaginálním porodu. V diskusi jsou všechny zjištěné problémy okomentovány.

Abstract:

This bachelor thesis deals with breastfeeding after the caesarean section. Correct breastfeeding techniques, factors affecting breastfeeding, breastfeeding position and most common breastfeeding problems are described in the first part. Differences between vaginal delivery and caesarean section, types of anesthesia and explanation of early puerperium are also mentioned in the theoretical part. The main aim was to determine the most common problems with breastfeeding after the cesarean section and differences of these problems between cesarean section and vaginal delivery. The data were collected by anonymous questionnaire, which was given to women who gave birth on the day and were the third day after birth. Totally data consist of 115 women. 81 of them were after vaginal delivery and 34 of them were after cesarean section. In the practical part of this thesis, some problems with breastfeeding were detected. One of the problems was the successful first application. It was shown that women after cesarean section were less successful at first application (53.0%) than women after vaginal delivery (36.3%). The most significant difference was in the feeding by artificial nutrition. 47.1% of women after the caesarean section fed their babies by artificial nutrition and only 17.5% of women after vaginal delivery used artificial nutrition. All identified problems are discussed in the discussion.
