Experiences of midwives in the support of women going through perinatal loss

Abstract:

Cíl: Cílem této práce bylo zmapovat zkušenosti porodních asistentek s podporou žen procházejících perinatální ztrátou. Popsat, jak porodní asistentky vnímají specifika komunikace s rodinou prožívající perinatální ztrátu, jak pomáhají rodičům rozloučit se se zemřelým dítětem, jak se porodní asistentky cítí připraveny na tento aspekt své profese, jak subjektivně prožívají setkání se smrtí dítěte a truchlícími rodiči a jakým způsobem se s tímto vyrovnávají. Metodika: Výzkumný soubor tvořilo pět kvalifikovaných porodních asistentek, které pracovaly minimálně dva roky na porodních sálech různých zdravotnických zařízení a mají zkušenost s péčí o ženu s perinatální ztrátou. Výzkum je koncipován jako kvalitativní šetření, data byla sbírána metodou polostrukturovaných rozhovorů. Výsledky: Pro porodní asistentky bylo náročné starat se o ženy s perinatální ztrátou, nebyly si jisté v komunikaci s nimi, prožívaly během péče intenzivní emoce, často shledávaly své zkušenosti jako nedostatečné a postrádaly další vzdělávání. Většina porodních asistentek také podceňovala péči o své psychické zdraví. Závěr: Vzhledem k poznatkům výzkumu by mohlo být rozšířeno pregraduální i postgraduální vzdělávání porodních asistentek v oblasti perinatální ztráty, a na pracovištích zavedena supervize a screening syndromu vyhoření. Abstract:

Objectives: The main objective of the thesis was the description of midwives' experiences with supporting women experiencing perinatal loss. Thesis describes how midwives percieve the specifics of communicating with a family experiencing perinatal loss, how they assist the meeting with the baby, levels of confidence of midwives to provide bereavement support, subjective experiences o midwives facing the death of a child and grieving parents and how midwives cope with this. Methodology: Explanatory batch consisted of five qualified midwives who had worked for at least two years in the labour wards of different institutions and had experience of caring for a woman with perinatal loss. The research was designed as a qualitative investigation. Data was collected by semi-structured interwievs. Results: Midwives found challenging to care for women with perinatal loss, they were unsure of how to communicate with those women, midwives experienced intense emotions during the care, they also often found their experience with perinatal loss insufficient and expressed the need for further eduction. Most midwives also underestimated the care of their mental health. Conclusions: In light of the research findings, more undergraduate and postgraduate training of midwives in perinatal loss could be provided, and supervision and screening for burnout syndrome could be introduced in institutions.
