Frequent episodic tension-type headache: comparison of methods between Trigger Points Therapy and self-treatment exercises.

Abstract:

"The frequent episodic tension-type headache: a comparison of methodologies between Trigger Points Therapy and exercises of self treatment" - Experimental Research. This thesis aims to compare two different methods of treatment for frequent episodic tension-type headache (CTTEF) in order to draw some useful guidelines for the therapist in the management of patients with CTTEF. The study will make use of patients of both sexes between 30 and 65 years of age, selected according to the IHS classification criteria for frequent episodic tensiontype headache. The study will be excluded subjects with disc protrusion, rheumatoid arthritis, pregnancy or cervical trauma suffered in the last 2 years. The study will examine two evenly distributed groups of 10 people each. The first group (Group A) was treated by applying compression techniques ischemicinhibition on active trigger points in the upper trapezius, splenius capitis, sternocleidomastoid, and suboccipital muscles. The second group (Group B) have been proposed stretching exercises of the muscles of the head and neck to be run independently with prior instruction from the therapist and subsequent revision during the treatment period. For patients in Group A protocol will be administered with a frequency of 2 sessions per week, however, patients in Group B will have to follow the exercises on alternate days supplied to him by the therapist. At the beginning of treatment were measured data: ROM (Range Of Motion) Cervical VAS (Visual Analogue Scale), MIDAS (migrane Disabilty Assessment Questionnaire), QOLS (Quality of Life Scale). Were measured again the same data at the end of treatment and at follow-up. The results showed that Group A had a greater improvement compared to Group B in the 4 weeks of treatment; however, the latter has continued to improve in the next month and at follow-up showed a greater increase compared to Group A, but with inferior results; possible explanation could be that the subjects of the group of selftreatment have obtained a better understanding, based onempirical basis, of their posture, but will require further prospective studies to establish their certainty. Abstract:

cefalea di tipo tensivo episodica frequente (CTTEF) allo scopo di trarre delle linee guida utili per il terapista nella gestione dei soggetti affetti da CTTEF. Lo studio si avvarrŕ di pazienti di entrambi i sessi tra i 30 e i 65 anni di etŕ, selezionati secondo i criteri di classificazione IHS per la cefalea di tipo tensivo episodica frequente. Dallo studio saranno esclusi soggetti con protrusioni discali, con artrite reumatoide, in gravidanza o con traumi cervicali subiti negli ultimi 2 anni. Lo studio prenderŕ in esame due gruppi equamente distribuiti di 10 persone ognuno. Il primo gruppo (Gruppo A) č stato trattato tramite applicazione di tecniche di compressione ischemica-inibizione sui trigger points attivi nei muscoli: trapezio superiore, splenio del capo, sternocleidomastoideo e suboccipitali. Al secondo gruppo (Gruppo B) sono stati proposti esercizi di stretching dei muscoli di capo e collo da eseguire in autonomia con previa istruzione da parte del terapista e successiva revisione durante il periodo di trattamento. Il trattamento sarŕ della durata di 4 settimane, con follow-up dopo 1 mese. Per i pazienti del Gruppo A il protocollo verrŕ somministrato con una frequenza di 2 sedute a settimana, invece, i pazienti del Gruppo di B dovranno seguire a giorni alterni gli esercizi fornitigli dal terapista. All?inizio del trattamento si sono misurati i dati di: ROM (Range Of Motion) cervicale, VAS (Visual Analogic Scale), MIDAS (MIgrane Disabilty ASsessment questionnaire), QOLS (Quality Of Life Scale). Sono stati misurati nuovamente gli stessi dati alla fine del trattamento e al follow-up. Dai risultati č emerso che il Gruppo A ha avuto un miglioramento maggiore rispetto al Gruppo B nelle 4 settimane di trattamento; tuttavia quest?ultimo ha continuato a migliorare nel mese successivo e ha dimostrato al follow-up un incremento maggiore rispetto al Gruppo A, seppur con risultati inferiori; possibile spiegazione potrebbe La cefalea di tipo tensivo episodica frequente: metodologie a confronto tra Trigger Points Therapy ed esercizi di autotrattamento 10 essere che i soggetti del gruppo di autotrattamento abbiano ottenuto su base empirica una migliore conoscenza della propria postura, tuttavia si richiedono ulteriori futuri studi per appurarne la certezza. In conclusione si puň affermare che i benefici ottenuti tramite l?intervento del terapista sono risultati migliori in tutti i dati finali; tuttavia gli esercizi di autotrattamento si sono dimostrati come un?utile risorsa da poter affiancare al trattamento principale.
