Mechanism and spread of antibiotic resistances in Salmonella

Abstract:

Rezistence k antimikrobiálním látkám představuje přirozenou odpověď mikroorganismů na změněné podmínky vnějšího prostředí. Většina genů pro rezistenci k antibiotikům je lokalizována na mobilních elementech (IS elementy, transpozony, integrony, plazmidy) a právě schopnost získat a začlenit do genomu tyto nové genetické determinanty je nejčastější příčinou vzniku rezistence. Schopností konjugace a horizontálním přenosem genů rezistence k antibiotikům jsem se zabývala u dvou sérovarů druhu Salmonella enterica, S. Enteritidis a S. Typhimurium. Kmeny těchto sérovarů jsou nejčastější příčinou salmonelóz u lidí a společně jsou zodpovědné za přibližně 98 % humánních infekcí. U sérovaru S. Enteritidis jsem analyzovala soubor izolátů fágového typu PT13 rezistentních k ampicilinu. Kmeny pocházely z lokální epidemie, která se objevila v několika regionech České republiky v průběhu letních měsíců roku 2004. Pomocí PCR byl u všech kmenů identifikován gen blaTEM, který byl lokalizován jako součást TnA transpozonu na konjugativním plazmidu pAMP200 o velikosti 200 kb. Při charakterizaci izolátů pomocí pulzní gelové elektroforézy (PFGE) byl potvrzen klonální původ všech epidemických kmenů a tyto kmeny byly odlišeny od skupiny antibioticky senzitivních kmenů stejného fágového typu pocházejících z let 1998-2003. U gram-negativních bakterií jsou se šířením genů rezistence spojovány tzv. R-plazmidy. U souboru multirezistentních kmenů S. Typhimurium jsem identifikovala konjugativní R-plazmidy, které byly zodpovědné za přenos genetických markerů antibiotické rezistence. U těchto plazmidů byly určeny geny rezistence zodpovědné za rezistentní fenotyp kmene a identifikovány tři různé typy integronů třídy jedna. V některých případech docházelo v průběhu konjugace ke ztrátám genů rezistence a u těchto kmenů byla identifikována oblast transpozonů Tn3 a Tn1721, která se vyštěpovala během konjugačního procesu. Početnou skupinu mezi rezistentními kmeny salmonel tvoří pentarezistentní kmeny S. Typhimurium fágového typu DT104. Rapidní nárůst těchto kmenů je spojován s celosvětovým rozšířením jediného klonu, který se poprvé objevil v roce 1984 ve Velké Británii. Pentarezistentní fenotyp u kmenů S. Typhimurium DT104 je podmíněn přítomností genetického elementu Salmonella Genomic Island 1 (SGI1). Pomocí PCR jsme u SGI1 pozitivních DT104 kmenů identifikovali deleci lokusu STM0517-0529 pro utilizaci alantoinu a tato delece byla též detekována u čtvrtiny SGI1 negativních kmenů. Pomocí metody PFGE a PCR pro specifické profágové sekvence byla ověřena příbuznost SGI1 negativních a SGI1 pozitivních kmenů S. Typhimurium, které ve svém genomu shodně postrádají oblast STM0517-0529. Z obdržených výsledků pak vyplynul možný evoluční vývoj kmenů S. Typhimurium. V původní populaci kmenů citlivých k antibiotikům se nejprve objevily kmeny s delecí genů pro metabolizmus alantoinu a následně v polovině 80. let některé z těchto kmenů začlenily do svého genomu SGI1 element.

Abstract:

Resistance to antimicrobial agents is a common response of microorganisms to the changing environmental conditions. Most of the antibiotic resistance genes are located on mobile elements (IS elements, transposons, integrons, plasmids) and the ability to acquire and integrate such genetic elements is the most common cause of the antibiotic resistance. In two Salmonella enterica serovars, S. Enteritidis and S. Typhimurium, the ability of conjugation and horizontal gene transfer was investigated. The strains of these serovars are the most frequent cause of human salmonellosis and are responsible for approximately 98 % of human salmonella infections. In S. Enteritidis, a collection of ampicillin resistant strains of phage type PT13 was analysed. The strains originated from several districts in the Czech Republic, where an increased incidence of these strains was recorded in summer 2004. Using PCR typing, the blaTEM gene was identified in all the strains and this gene was localised on the TnA transposon on the conjugative plasmid pAMP200 of 200 kb in size. Using pulsed-field gel electrophoresis (PFGE), the clonal origin of all epidemic PT13 strains was confirmed and these strains were easily distinguished from the antibiotic sensitive strains of the same phage type isolated between 1998-2003. In gram-negative bacteria, the spread of antibiotic resistance markers is mainly associated with R-plasmids. In a collection of multidrug resistant S. Typhimurium strains, R-plasmids responsible for conjugational transfer of antibiotic resistance markers were identified. The appropriate antibiotic resistance genes responsible for the resistance phenotype of each strain were determined and altogether three types of class one integrons were detected. In 2 cases, the loss of antibiotic resistance genes was observed. The instability was associated with transposons Tn3 and Tn1721. The pentadrug resistant S. Typhimurium strains of definitive phage type DT104 represent a special group of multidrug resistant strains of the Salmonella genus. The rapid increase of these strains is associated with the world-wide spread of a single clone which first appeared in the Great Britain in 1984. The pentadrug resistant phenotype in S. Typhimurium DT104 strains is caused by the presence of a specific genetic element, Salmonella Genomic Island 1 (SGI1). Using PCR, the deletion of the STM0517-0529 locus coding for the anaerobic utilisation of allantoin was identified in all SGI1 positive DT104 strains. This deletion was also detected in 24 % of SGI1 negative S. Typhimurium strains. Using PFGE typing and PCR detection of specific prophage sequences, the SGI1 positive and negative strains lacking the STM0517-0529 locus were determined to be highly genetically related. The results obtained by the PFGE and PCR typing lead to the proposal of a recent evolution of S. Typhimurium strains. First, the original population of antibiotic sensitive strains which were able to utilise allantoin lost the STM0517-0529 locus and then in mid 1980s a strain of this lineage acquired the SGI1 element.
