Vývoj protrombotického stavu u HIV-pozitivních pacientů a jeho vztah k dalším metabolickým, imunologickým a virologickým markerům

The development of prothrombotic state in HIV-positive patients in relation to other metabolic, immunological and virological markers

Abstract:

Zavedením efektivní antiretrovirové terapie došlo u HIV-pozitivních osob k významnému poklesu výskytu AIDS definujících komorbidit, což významně modifikovalo prognózu onemocnění i očekávanou délku přežití. S prodlužující se délkou života je však u HIV-pozitivních osob pozorována akcelerace stárnutí a předčasný výskyt neinfekčních tzv. non-AIDS onemocnění, což je rozsáhlá skupina chorob, která je u HIV-negativní všeobecné populace typická pro vyšší věk a stárnutí (zejména kardiovaskulární choroby, metabolický syndrom, různá degenerativní a nádorová onemocnění, neurokognitivní poruchy a další). U HIV-pozitivních jedinců se tyto choroby objevují v podstatně nižších věkových kategoriích. Stárnutí a s ním spojené patologické stavy jsou obvykle provázeny známkami oxidačního stresu, protrombotického stavu a zánětu. Studie u HIV-pozitivních pacientů z hlediska alterace různých markerů těchto procesů dávají kontroverzní závěry, není konsensus, u kterých parametrů dochází k alteraci, u kterých nikoli, případně není jasná prediktivní síla alterovaných parametrů. V naší studii jsme se zaměřili na experimentální vyšetření markerů souvisejících s oxidačním stresem (malondialdehyd, glutathion, nízkomolekulární antioxidanty) a protrombotickým stavem (mikropartikule, e-selektin). Experimentální hodnoty byly dány do souvislosti s klasickými imunologickými, virologickými a metabolickými markery a porovnány mezi HIV-pozitivními jedinci s antiretrovirovou léčbou, bez antiretrovirové léčby a s kontrolní HIV-negativní skupinou. U HIV-pozitivních osob léčených antritretrovirovou terapií bylo v naší studii prokázáno signifikantní zvýšení známek oxidačního stresu, prokoagulační stav s endotelovou aktivací a alterace lipidového spektra proaterogenního charakteru, které predisponují k expresi fenotypu imunosenescence a vzniku nemocí typických pro vyšší věk a stárnutí.

Abstract:

The introduction of effective antiretroviral therapy has led to a significant decrease in the incidence of AIDS-defining comorbidities in HIV-positive individuals, which has considerably improved the prognosis of the disease and the life expectancy. However, with the increasing life expectancy, an acceleration of aging and premature onset of non-infectious non-AIDS diseases were observed in HIV-positive persons. These conditions are otherwise typical of advanced age in the HIV-negative population (especially cardiovascular diseases, metabolic syndrome, various degenerative diseases, malignancies, neurocognitive disorders and others). In HIV-positive people, these conditions occur at a significantly younger age. Aging and related pathological conditions are usually associated with signs of oxidative stress, prothrombotic state and inflammation. Existing research on HIV-positive patients that investigated alterations of various markers of these processes gives controversial conclusions. No consensus on the significance of altered parameters has been reached and its predictive power is not clear as well. In the present study, we focused on the experimental examination of markers associated with oxidative stress (malondialdehyde, glutathione, low molecular weight antioxidants) and prothrombotic state (microparticles, E-selectin). Experimental levels were compared with common immunological, virological, metabolic markers and the groups of HIV-positive individuals with or without antiretroviral therapy were compared to the HIV-negative controls. In HIV-positive persons treated with antiretroviral therapy, this study demonstrated a significant increase in markers of oxidative stress, procoagulant status with endothelial activation, and proatherogenic lipid spectrum alterations. These conditions predispose to the expression of an immunosenescent phenotype and the development of diseases typical of advanced age.
