Effect of succesful endovascular treatment of aortic dissection Stanford type B on aortic size and scaling of the true and the false lumen

Abstract:

Hlavním cílem disertační práce je na selektovaném souboru pacientů léčených implantací stentgraftu do oblasti primárního entry objasnit způsob hojení a remodelace aorty po provedené úspěšné endovaskulární léčbě a potvrdit prognosticky příznivý vývoj a stabilitu aorty ve všech jejích anatomických segmentech. Základní soubor, nashromážděný v letech 2000 až září 2010 a hodnocený v naší studii, zahrnuje celkem 27 pacientů léčených endovaskulární způsobem pro disekci aorty typu B a 14 pacientů, kteří byli indikováni ke konzervativnímu postupu léčby Naším sledováním se nám podařilo prokázat pozitivní vliv úspěšné endovaskulární léčby překrytím primárního entry na stabilizaci a remodelaci aorty pro akutní i chronické stadium choroby ve střednědobém horizontu sledování. Překrytím primárního entry a navozením trombosy false lumen dochází ke statisticky významnému zmenšení celkového průměru hrudní aorty s nárůstem velikosti pravého a zmenšováním velikosti falešného kanálu.

Abstract:

The main aim of the thesis is to detect group of patients treated with implantation of the stent graft within the primary entry to clarify the way of healing and remodeling of the aorta after a successful endovascular treatment and prognosis to acknowledge the positive development and stability of the aorta in all its anatomical segments. Population, accumulated from 2000 to September 2010 and evaluated in our study includes a total of 27 patients treated with endovascular method for aortic dissection type B and 14 patients who were indicated to conservative therapies. Our monitoring we were able to demonstrate the positive impact of successful endovascular treatment of the primary entry to the superimposition of stabilization and remodeling of the aorta for both acute and chronic stage of disease in the medium-term monitoring. Overlapping primary entry and inducing false lumen thrombosis occurs in a statistically significant reduction in the overall average increase in thoracic aorta with the right size and decreasing the size of the false channel
