Abstract:

V této bakalářské práci se zabývám tématem adaptivní radioterapie. Má práce je rozdělena do několika částí. V první části popisuji důležitost verifikačních systémů, které jsou nezbytné pro zajištění kvalitní a bezpečné práce, zajišťují správné a přesné nastavení pacienta při každodenním ozařování a sledují frakcionaci, denní dávku a přesnou distribuci dávky do ozařovaného objemu. Dále se zabývám metodou IGRT a IMRT, které jsem věnovala samostatnou kapitolu. Úkolem těchto metod je co nejpřesněji zaměřit cílový objem a ten, aby měl co nejmenší okraje. Tím se dosáhne většího šetření zdravé tkáně. Další kapitolu jsem věnovala problematice nádorů hlavy a krku. Popisuji zde nádory u kterých je nutné, vzhledem k anatomickým strukturám, po určitém čase přeplánování. A také, u kterých nádorů to nutné není. V další kapitole definuji možnosti, jak nejvíce zpřesnit radioterapii. Popisuji zde možnosti dosažení co největší preciznosti v radioterapii, včetně nejnovějších postupů využívajících dynamické zobrazování během ozařovacího cyklu. Poslední část mé práce představuje vlastní studie, ve které se zabývám změnou objemů krků u pacientů. Srovnávám zde změny objemů po desáté a devatenácté frakci a porovnávám je se stanovenou mezní hodnotou.

Abstract:

In this bachelor thesis I consider with the topic of adaptive radiotherapy. My work is divided into several parts. The first section describes the importance of verification systems that are necessary to ensure quality and safety work, ensuring accurate and precise adjustment of the patient during daily fractionated irradiation and monitored, daily dose and distribution of its benefits to the irradiated volume. Another part deals with the method of IGRT and IMRT, which I have devoted a separate chapter. The objective of these methods is to focus as closely as possible the target volume and to have the smallest margin. This enables a larger investigation of healthy tissue. Next chapter I apply on the issues of head and neck cancer. I describe tumors which are necessary, due to anatomical structures, after a certain time replanning. And also, where the tumor is not necessary. In the next chapter I define the ways of the most refined radiotherapy. Here I describe the possibility of achieving the greatest accuracy in radiotherapy, including the latest procedures using dynamic imaging during the irradiation cycle. The last part of my thesis is its own study, which deals with the change in volume in patients neck. Here I compare the changes in volume after ten and nineteen fractions and compare them to a specified limit.
