Psychological characteristics of patients with pyrosis - systematic retrieval

Abstract:

Cíle: V této práci se věnujeme psychologickým aspektům osob s chronickým pálením žáhy a pomocí systematické rešerše s meta-analytickým zpracováním výsledků objasnit, jaké osobnostní charakteristiky a strategie zvládání stresu mají spojitost se vznikem nemoci a ve kterých se jednotlivé diagnózy liší mezi sebou. Pro zodpovězení otázek byla provedena systematická rešerše za účelem meta-analytického zpracování. Metoda: Nemocí projevujících se chronickou pyrózou je více, v práci jsou vysvětleny jejich rozdíly a úskalí správné diagnostiky. Dále je představeno mnoho souvisejících teorií a pojmů z psychosomatiky, vlivu stresu na organismus, strategií zvládání a osobnostní charakteristiky. Na základě systematického vyhledávání v elektronických databázích EBSCO, Scopus a Web of Science bylo nalezeno 3180 studií, z nichž 9 studií (publikovaných v letech 2003-2018) odpovídalo tématu pyrózy a zvoleným kritériím. Ty byly důkladně prozkoumány z hlediska možnosti statistického zpracování a výsledků, které získaly. Výsledky: Žádná studie věnující se copingovým strategiím nebyla nalezena. Devět nalezených studií přináší porovnání pacientů s různými gastroezofageálními refluxními chorobami pomocí metod Symptomt Checklist 90, Minnesota Multiphasic Personality Inventory a jejich revidovaných verzí a Spielbergerovy STAI. Závěr: Ze získaných informací nelze vyvodit jednoznačné závěry, pouze částečně potvrdit dosavadní poznání vlivu tendencí pacientů k somatizaci. Bylo by potřeba ve studiích důkladnější diferenciální diagnostiky, prozkoumat více probandů a pro nezkreslené výsledky sledovat intervenující komorbidity funkčních poruch zažívacího traktu.

Abstract:

Aims: In this study we focus on psychological traits of persons with chronical heartburn. We try to clarify which personality characteristics and coping strategies are associated with the onset of chronical heartburn and if there are any differences in them reagarding particular disorders. To answer these questions we carry out systematic research for meta-analytic purposes. Method: There are many diseases manifesting themselves as heartburn. In this research we explain differences between them and problems of giving correct diagnosis. Terms and theories of psychosomatic medicine, impact of stress on organism, coping strategies and personality characteristics are presented. Based on systematic search in electronic databases EBSCO, Scopus and Web of Science we identified 3180 papers related to GERD topic from which 9 studies (published from 2003-2018) matched the topic and set criteria. These 9 studies were thoroughly investigated from the point of view of statistical processing and results obtained. Results: No study focused on coping strategies at GERD patients was found. The data from selected nine papers enabled comparison of different GERD types based on the Symptom Checklist 90, Minnesota Multiphasic Personality Inventory and their revised versions and Spielberger’s State Trait Anxiety Inventory. Conclusion: A clear conclusion can not be drawn from the information retrieved. However, the existing understanding of the influence of patient tendencies on somatization can be partly confirmed. For better knowledge of chronical heartburn causes it is needed to have studies with thorough differential diagnostics, larger research sample and for undistorted results it is necessary to pay attention to intervening functional comorbidities of the gastrointestinal tract.
