Importance of the pelvic floor treatment in L4-L5 e L5-S1 disc herniation diseases: Pilot study.

Abstract:

L?importanza del trattamento del pavimento pelvico nelle patologie di ernie discali L4-L5 e L5-S1: Studio Pilota ? The Herniated Nucleus Pulposus is a very common pathology, which manifests itself in many different forms, motivating the development of multiple therapeutic options, albeit not all of equivalent value. Among them, non-surgical interventions occupy a relevant position. The rehabilitating approach based on Back School model consists of a systematic, yet also flexible rehabilitation program directed to patients with vertebral pathologies, including those related to herniated nucleus pulposus. This approach is based on mechanisms of evaluation, training, and prevention, which can be personalized according to the specific pathology and functional deficit of the patient. The back school is an example of multidisciplinary treatment, which combines an educational, kinesitherapy, rehabilitating, and psychological component. In order to further improve the results obtained using this approach, it is important to understand whether passive work on the pelvic floor, when applied to patients with level radiculopathy, could shorten the healing time, expedite the functional recovery of the spine, and above all improve the quality of the daily life. The goal of this study is to demonstrate that it is possible to obtain better results in the treatment of herniated nucleus pulposus, by combining the back school approach with passive rehabilitation of the pelvic floor. A first cohort of 20 patients (50% men, 50% women) have been randomly divided in two group. In group A, patients were subjected to a treatment consisting of 9 treatments, which in addition to involving the rehabilitation procedure according to the Back School model, included also passive rehabilitation of the pelvic musculature. The patients in group B were only subjected to the same number of treatments but using only the rehabilitation method according to the Back School model. The VAS and the Active Range of Motion were evaluated at different times. For both groups, an evaluation was made at time 0 (T0), i.e. prior to the treatment, and at time T1, i.e. immediately after treatment of the pelvic musculature and exercise according to the _____________________________________________________________________________ 18 L?importanza del trattamento del pavimento pelvico nelle patologie di ernie discali L4-L5 e L5-S1: Studio Pilota. Back School. These evaluations were made at each session of treatment. Finally, the patients were evaluated after four weeks from the end of the treatment cycle in order to examine their pain perception in the medium-long term. To analyze the effect of the treatment with respect to each variable, the method ANOVA of type I (Variance analysis) was utilized. Statistical analysis of the results reveled significant positive effects for the variable VAS. Indeed, the patients who had received passive work on the pelvic musculature have experienced significant relief from the pain at shorter times. The trend across the groups was significant also with respect to the range of articular flexion. Finally, the follow-up evaluation at 4 weeks from the treatment revealed that the patients who had received passive work on the pelvic musculature also show more positive and longer-lasting effects as compared to those treated only using the Back School model. The conclusion from this study is that patients that receive passive treatment of the pelvic floor show, both in the short- and in the medium-term, better results with respect to pain relief and improvement of the range of articular flexion.

L?ernia del disco č una patologia molto frequente, che puň rappresentare manifestazioni molto diverse e che ha incentivato lo sviluppo di molteplici opzioni terapeutiche, di valore ineguale, al cui intento le metodiche non chirurgiche occupano una posizione di rilievo. L?approccio riabilitativo che segue il modello della Back School, consiste in un programma di riabilitazione sistematico ma al contempo flessibile rivolto a pazienti con disturbi vertebrali, compresi quelli con ernia del disco. Tale approccio si basa su meccanismi di valutazione, educazione e prevenzione, in quanto puň essere personalizzato in funzione della specifica patologia e dello specifico deficit funzionale del paziente. La Back School č un esempio di trattamento multidisciplinare, al contempo educativo, cinesiterapico, riabilitativo e psicologico. Al fine di migliorare ulteriormente i risultati ottenuti utilizzando la metodica sopra citata, č interessante capire se un lavoro passivo della muscolatura pelvica, applicato a pazienti con radicolopatia a livello di L4-L5 e L5-S1, possa accorciare i tempi di guarigione, velocizzare la ripresa funzionale del rachide lombo-sacrale e soprattutto migliorare la qualitŕ della vita quotidiana. L?obiettivo di questo studio mira a dimostrare che č possibile ottenere risultati di maggiore successo nel trattamento di un?ernia discale L4-L5 e L5-S1, associando al modello della Back School una riabilitazione passiva del pavimento pelvico. Venti pazienti iniziali (10 donne e 10 uomini) sono stati assegnati, a random seguendo il criterio del sorteggio, a uno dei due gruppi di studio: coloro i quali appartenevano al Gruppo A sono stati sottoposti ad un trattamento di 9 sedute, che comprendeva oltre alla metodica riabilitativa secondo il modello della Back School, anche una riabilitazione passiva della muscolatura pelvica; quelli che appartenevano al Gruppo B, invece, sono stati sottoposti alla sola metodica riabilitativa secondo il modello della Back School, sempre per 9 sedute. Sono stati valutati la VAS e il Range of Motion Attivo in flessione di rachide in diversi tempi: per entrambi i gruppi di studio, č stato misurato un _____________________________________________________________________________ 16 L?importanza del trattamento del pavimento pelvico nelle patologie di ernie discali L4-L5 e L5-S1: Studio Pilota. tempo T0 concernente la valutazione prima dell?inizio del trattamento e un tempo T1 relativo alla valutazione immediatamente dopo il trattamento alla muscolatura pelvica e alla ginnastica secondo la Back School. Tutte le valutazioni sono state eseguite durante ogni seduta. Una volta terminato il ciclo di trattamento, i pazienti sono stati richiamati 4 settimane dopo per avere una valutazione a medio-lungo termine della loro sensazione dolorifica. E? stato utilizzato il metodo ANOVA di Tipo I (Analisi della Varianza) per esaminare gli effetti del trattamento per ogni variabile. Lo studio statistico dei risultati ha evidenziato esiti significativi e positivi per la variabile VAS: i pazienti che hanno ricevuto un trattamento passivo della muscolatura pelvica hanno ottenuto un notevole miglioramento della sensazione dolorifica in tempi inferiori. L?interazione tra i gruppi č stata significativa anche per il range articolare in flessione. Anche il follow-up dopo 4 settimane ha riscontrato che i pazienti che sono stati trattati passivamente alla muscolatura pelviva mostrano risultati migliori e piů duraturi rispetto a chi č stati trattato solo con la ginnastica secondo il modello della Back School. Quindi, i pazienti a cui č stato trattato passivamente il pavimento pelvico mostrano risultati significativi nella riduzione del dolore e nel miglioramento del range articolare in flessione, sia nel breve sia nel medio-lungo termine.
