Modern trends in the treatment of varicose veins in the lower limbs

Abstract:

Cílem předkládané doktorské disertační práce je ukázat současné možnosti ambulantní operace varixů na principu tzv. jednodenní chirurgie miniinvazivními metodami, se zaměřením především na endovenozní operace laserem a radiofrekvenční energií. Využití endovenozních metod při operaci kmenových i kolaterálních varixů otevírá další možnosti zvýšené radikality léčby při minimální agresivitě zákroku a maximálním léčebném i kosmetickém profitu pro pacienta. Nezanedbatelný je rovněž i ekonomický benefit těchto ambulantních metod. Vstupní investice - laserový či radiofrekvenční generátor – jsou sice vyšší, ale léčba endovenozními metodami bez nutnosti hospitalizace, prakticky bez nutnosti pracovní neschopnosti a bez omezení pacienta v běžném životě a minimalizováním komplikací jednoznačně ukazují směr, kterým se léčba varixů bude ubírat. V práci jsou porovnány tyto metody a je poukázáno na výhody endovenozníchh operačních výkonů u varixů dolních končetin. Dílčím cílem bylo i navržení určitého algoritmu pro ambulantní miniinvazivní operace varixů.

Abstract:

The aim of the submitted doctorate dissertation work is to show the current possibilities of out-patient varicose vein operations on the principle of so-called one-day surgery using mini-invasive methods, with the intention above all on endovascular laser operations and radio-frequency energy. The use of the endovascular method during operations on main and collateral varicose veins opens further possibilities to increase the radical treatment with minimal aggressive surgery and the maximum medical and cosmetic benefit to the patient. Undoubtedly, it balances out with the economical benefits of this out-patient method. The initial investment – laser or radiofrequency generator – is rather high, but treatment by the endovascular method without the need for hospitalisation, practically no inability to work, and without changes to the patient s everyday life, and the minimalisation of complications, singularly points out the way that treatment of varicose veins will take. These methods have been compared in practice and the results of endovascular operation on varicose veins in the lower limbs are shown to be advantageous. A secondary aim was also the suggestion of definite algorithms for out-patient mini-invasive surgery of varicose veins.
