Resistome of hospitalized patients with inhalation injury

Abstract:

Inhalační trauma (INHI) vzniká v důsledku inhalace horkých par, zplodin hoření a případně toxických látek. Většina pacientů však není přímo ohrožena primárním poraněním nebo účinkem inhalovaných látek, ale spíše sekundárními komplikacemi, především těmi infekčními. Problémem léčby bakteriálních infekcí je antibiotická rezistence, která souvisí s přítomností genů antibiotické rezistence (ARG). Cílem této diplomové práce bylo: i) zavést metodiku multiplex qPCR pro identifikaci vybraných bakterií a ARG a, ii) použít oba multiplex qPCR panely při analýze vzorků z dolních cest dýchacích a moči od pacientů s INHI v průběhu jejich hospitalizace, a iii) stanovit v těchto klinických vzorcích celkovou bakteriální nálož pomocí qPCR. Výběr tří genů pro stanovení bakterií (Klebsiella pneumoniae; Staphylococcus aureus; Pseudomonas aeruginosa) a tří ARG (blaTEM; sul1; tetM) byl proveden na základě literární rešerše a s ohledem na mikrobiologický nález ve studovaném souboru. Pro zavedení multiplex qPCR panelů byly použity pozitivní kontroly, tedy mikrobiologicky vyšetřené klinické vzorky mimo studovaný soubor. Specifické bakteriální geny, ARG a celkovou bakteriální nálož jsem stanovila pomocí dvou panelů a qPCR ve 30 vzorcích bronchoalveolární laváže (BAL) a 32 vzorcích moče odebraných od 10 pacientů s INHI během jejich hospitalizace (1., 7., 14. a případně i 28. den hospitalizace). Byla úspěšně zavedena multiplex qPCR pro identifikaci ARG a specifické bakteriální geny pro S. aureus a P. aeruginosa. Účinnost amplifikace se pohybovala v rozmezí 93-111 % a hodnoty kalibračních křivek R2 blízké 1 představují relativně konzistentní a reprodukovatelné výsledky. Pomocí detekce ARG byly zjistěny v některých vzorcích BAL a moče predispozice k rezistenci bakterií na beta-laktamová antibiotika, sulfonamidy i tetracykliny. K. pneumoniae se nepodařilo začlenit do multiplex qPCR panelu, alternativou je vybrat do budoucna jiný cílový gen než khe. Analýza BAL prokázala přítomnost P. aeruginosa u jednoho pacienta, zatímco ve vzorcích moče nebyla přítomnost detekována vůbec. S. aureus nebyl identifikován ve vzorcích BAL, ani ve vzorcích moče. U konkrétních pacientů s INHI a časových bodů hospitalizace nálezy z genotypizace odpovídají v převážné většině nálezům z fenotypizace. Výsledky analýzy detekce ARG by tak bylo hypoteticky možné zohlednit při nasazování antibiotik u hospitalizovaných pacientů s INHI. Nicméně pro komplexnější charakterizaci AR je vhodné využívat kombinaci genotypických a fenotypických metod.

Abstract:

Inhalation trauma (INHI) is caused by inhalation of hot vapours, combustion products and possibly toxic substances. However, most patients are not directly at risk from the primary injury or the effect of inhaled substances, but rather from secondary complications, especially infectious ones. A problem in the treatment of bacterial infections is the antibiotic resistance of bacteria, which is related to the presence of antibiotic resistance genes (ARGs). The aims of this thesis were to: i) establish a multiplex qPCR methodology for the identification of selected bacteria and ARGs, ii) use both multiplex qPCR panels in the analysis of lower respiratory tract and urine samples from patients with INHI during their hospitalization, and iii) determine the total bacterial load in these clinical samples by qPCR. The selection of three genes for bacterial detection (Klebsiella pneumoniae; Staphylococcus aureus; Pseudomonas aeruginosa) and three ARGs (blaTEM; sul1; tetM) was based on a literature search and concerning the microbiological findings in the study population. Positive controls, i.e., microbiologically examined clinical samples outside the study set, were used to establish multiplex qPCR panels. Specific bacterial genes, ARGs and total bacterial load were determined using two panels and qPCR in 30 bronchoalveolar lavage (BAL) samples and 32 urine samples collected from 10 patients with INHI during their hospitalization (day 1, 7, 14 and 28 of hospitalization, if applicable). Multiplex qPCR was successfully established to identify ARGs and specific bacterial genes for S. aureus and P. aeruginosa. Amplification efficiencies ranged from 93-111 % and R2 values of the calibration curves close to 1 represent relatively consistent and reproducible results. Using ARG detection, some BAL and urine samples were found to predispose the bacteria to resistance to beta-lactams, sulfonamides and tetracyclines. K. pneumoniae failed to be included in the multiplex qPCR panel; an alternative is to select a target gene other than khe in the future. BAL analysis showed the presence of P. aeruginosa in one patient, while no presence was detected in urine samples. S. aureus was not identified in BAL or urine samples. For specific patients with INHI and time points of hospitalization, genotyping findings were overwhelmingly consistent with phenotyping findings. Thus, the results of the ARG detection analysis could hypothetically be considered in antibiotic deployment in hospitalized patients with INHI. However, for a more comprehensive characterization of AR, it is advisable to use a combination of genotypic and phenotypic methods.
